Care Neighborhood (CN) Enhanced Care Management (ECM) is an innovative program to support and empower identified high-risk, high-needs patients to improve their health outcomes. The multidisciplinary team works together to support patients and address the social drivers that impact health, such as food, housing, transport, and financial resources.

The Community Health Worker (CHW) will work primarily with the clinic’s patients.  There will be a period of orientation with the central team and within TVHC. After this, the CHW will work in the clinic and in the field. Activities include outreach and engagement, development of the care plan, linkage of the patient with resources (food, housing, transport, financial) as well as linking the patient with care within the clinic. Ideally, the CHW will be a trusted member of the community in which they will work.  This relationship will enable the CHW to serve as a liaison between health and social services and the clinic.  

This is a full-time position working 40 hours per week, typically Monday through Friday.

Tiburcio Vasquez Health Center is a non-profit community health center that is dedicated to promoting the health and well-being of our community by providing accessible, high-quality care by integrating primary care, dental care, WIC support, mental health counseling, community health education, and more.

Compensation: $21.41 - 23.40 per hour, depending on experience.

Responsibilities:

  • Advocate for patients and help them achieve their best health.
  • Be highly dependable and independent – much of the work will take place in various settings – medical office, specialists, hospitals and patients’ homes. 
  • Communicate well with patients, document clearly in the medical record and interface with medical providers to advocate for the patient.
  • Connect patients to resources to meet their needs.
  • Serve as a liaison between the community and health and social services.
  • Interface well with clinic staff, community and Care Neighborhood team.
  • Meet departmental review and documentation standards for work assignment.
  • Adhere to the policy and procedures of TVHC
  • Build and maintain appropriate relationships
  • Support a positive work environment and foster teamwork.Ability to de-escalate tense situations; training in de-escalation provided
  • Must have the ability to maintain professional boundaries, including adhering to confidentiality, while building trusting relationships with patients, and staff.
  • Conduct outreach, enrollment assessments, and care plans that are person-centered.
  • Schedule health-related transportations and other social supports that may be barriers to access to care.
  • Employ evidence-based behavior change strategies that promote and encourage positive behavior change.
  • Report incidences of child or elder abuse, neglect, or threats of harm to authorities, as required.
  • Maintain full caseload and complete program requirements to help patients achieve their best health
  • Other duties as requested by supervisor

Requirements

  • Bilingual in English and Spanish required.
  • BLS certification required.
  • Bachelor’s degree or a combination of High school diploma/AA with 3 years in a community-based health care service capacity.
  • At least three (3) years of progressively responsible work experience in a community-based health care service capacity.
  • Strong understanding of the managed care environment.
  • Ability to work effectively and collaboratively with a variety of customers including hospital/office staff, health plans, members and physicians.
  • Travel to work assignments, must have a valid driver's license.

Qualifications:

  • Competent leadership and administrative skills.
  • Good communication and customer relation skills; ability to work well within a team and independently.
  • Proficiency in computer operations, navigational skills, and comfortable with Internet-based applications. Experience with electronic health records preferred
  • Sound decision-making skills including problem-solving, critical thinking, and good clinical judgment for clinical and non-clinical issues.
  • Professional demeanor.
  • Flexible and adaptable to change.
  • Commitment to working directly with low-income communities with diverse backgrounds, in a helpful, supportive manner.
  • In-depth knowledge of and proficiency in case management, health issues, and the health care system.
  • Ability to learn quickly and retain complex information.
  • Excellent communication, organizational, and time management skills with the ability to meet tight timeframes.
