Job Summary:

Responsible for exemplifying the UT Tyler Health Science Center's Mission, Vision and Values, providing outstanding customer service the Community Health Worker (CHW) Apprentice will function as a team member to assist with more than one project by providing patient/family support to help community members and patients to achieve good health practices, and/or community outreach. Primary responsibilities include assistance with facilitation of health improvement, health promotion, coalition development and support, health prevention activities, and coordination with regional programs and other partners within the community. The CHW apprentice will successfully complete the Texas Department of State Health Services CHW certification and the Department of Labor CHW Apprenticeship program.

Major Responsibilities / Duties / Critical Tasks:

  • Complete timely documentation and required reporting for all services in appropriate template.

  • Participate in community outreach and assist clinical staff with health screenings in the community.

  • Provide one-on-one education and encourage healthy parenting and behaviors with patients.

  • Conduct patient home visits, when necessary, i.e.., follow-up, health education, application assistance, etc.

  • Assess and assist patient for potential eligibility for grants, programs, and/or community resources.

  • Assist clients in completion of forms/applications required to receive needed services.

  • Assist patients in scheduling future appointments and linking to providers and/or needed services.

  • Provide families with referrals to external community resources (such as food pantries) as needed.

  • Collaborate with other team members and providers to promote population health.

  • Handle all phone calls, emails, and in person interactions cordially and professionally.

  • Remain current on chronic disease and behavioral health trends, business practices, and health education and promotion theory.

  • Communicates effectively with team, The Center and UTT HSC colleagues, project partners, stakeholders, subcontractors, and consultants to achieve outcomes, metrics, and improvement targets.

  • Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; participating in professional societies as deemed appropriate.

  • Handle confidential/sensitive information with discretion and adherence to HIPAA rules.

  • Performs other duties as assigned.

  • Ability to work independently in an ambiguous environment; intuitive and proactive.

  • Working knowledge of Microsoft Office Products (Office, PowerPoint, Excel).

Required Education / Experience:

  • High School diploma or GED required.

  • One (1) year of clinical and/or healthcare related experience preferred.

  • One (1) year of experience conducting and promoting health education preferred.

  • Basic knowledge of standard office equipment such as: telephone, facsimile, copier, personal computer, printer and audio/visual.

  • Basic knowledge of health education and/or chronic disease prevention and management.
