POSITION SUMMARY

Community Health Care Workers (CHW) serves the community as a bridge between the member (community) and the healthcare system (providers) through outreach and education. By providing these services, CHWs help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties to become more acculturated with each other. This position will work under the direction of a designated professional

Fundamental Components:

Fundamental Components of the Community Health Worker include, but are not limited to:

  • Conduct home visits with Aetna members with chronic conditions to reinforce following the care plan, adjusting tasks to fit with the members culture, language, and religion, improve access to care, identify health care needs, and reduce unnecessary hospitalizations.

  • Educate healthcare professionals and providers on opportunities for improving and understanding the social determinants of health that may be impacting members health and treatment plan through various communication channels.

  • Build and maintain positive working relationships with the Aetna members, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds. Work to reduce cultural and socio-economic barriers between clients and institutions.

  • Assist with care plan implementation and Aetna member education during in-home visits, help develop care management strategies, and work with team members to provide linkages for the various health and social needs of members.

  • Assist members to identify socio-economic issues that affect their overall health and develop health/social management plans and goals.

  • Identify gaps and opportunities to strengthen systems of care within the members community and assists members in utilizing community services, including scheduling appointments with social services agencies, (including transportation vendors), and assisting with completion of applications for eligible programs.

  • Assist with patient medication adherence by: instructing the member on current medication list, reviewing medications with member and assist in obtaining refills.

  • Teach disease self-management (i.e. nutrition, symptom tracking and reporting).

  • Accompany members to appointments as needed.

  • Enter and maintain member records in electronic health record system, compile reports and complete other program documentation in a timely manner (e.g. progress notes, letters), and other administrative responsibilities as needed.

  • Travel as required and use of personal vehicle to client homes, community locations, various agencies, and other outreach destinations

BACKGROUND/EXPERIENCE desired:

  • 1-2 years experience providing clinical services; experience in community/outpatient setting preferred.

  • Community Health Worker Certification or equivalent training and experience preferred.

  • 3-5 years relevant experience.

  • Experience serving Medicaid populations in urban or rural environments; familiarity of local formal and informal resource networks preferred.

  • Bilingual skills desired.

EDUCATION

The minimum level of education desired for candidates in this position is a High School diploma, G.E.D. or equivalent experience.

The Family Services Worker provides services to individuals and/or families experiencing or at risk of experiencing abuse, neglect or exploitation, out of home placement, foster care, or seeking family reunification. Hiring Range for Family Services Worker I: $37,337 - $42,937 Hiring Range for Family Services Worker II: $44,339 - $50,989 Essential Functions Essential functions include but are not limited to: Family Services Worker I/II * Provides information to public about eligibility requirements and available services and resources for assigned area. * Conducts interviews and/or investigations. * Provides assessment of individual or family needs by developing and implementing service plans to address the needs of children, the elderly and/or disabled adults. * Formulates and implements service plans. * Manages cases and maintain records by preparing reports, recording all contact with clients. * Initiates request for client goods and services, requests approval and authorization, confirms provider availability and coordinates services. * Interacts/coordinates with other agencies, organizations or family/community members to provide services, makes referrals for other needed services. * Conducts home visits as directed to observe, monitor, and document family interactions, activities, and environment. * Provides direct services to individuals/families to include parenting education and support, budgeting, employment skills, conflict resolution, crisis intervention, accompanying and/or transporting individuals to appointments. * Participates in court hearings by providing written reports, recommendations and verbal testimony as required. * Collects and compiles information/data and prepares periodic and special reports as required. Maintain files and/or records for area of responsibility. * Attends meetings and/or conference to present cases, receive supervision and analyze services provided. * Develops knowledge of community resources to make appropriate referrals for clients and participates in training to further develop knowledge or skills. * Performs routine office tasks such as dictating case notes, data entry, typing correspondence for forms, filing, faxing, telephoning, or photocopying. * Performs other job duties, requiring skills, knowledge and physical requirements as demanded by those duties described or less. Possession of a BSW or MSW degree and a Commonwealth of Virginia Social Worker license are desirable.
