NOTE: The California Department of Public Health (CDPH) has issued new requirements for health care and congregant care facilities to decrease the risk of COVID-19 outbreaks. Community Health RN - This is a non-exempt position, 30 hours a week with (10 hours spent with the Psychiatry team and 20 hours with the Kinship team). The position is community based with home visits; the office is located in our South San Francisco office.
Program Description
The Community Health RN is responsible for the provision of health and education services for participants in the Edgewood Kinship program and to support the Psychiatry Team.
Essential Functions-Psychiatry
The community health RN will provide nursing care to psychiatry patients and support for the department caseload including but not limited to:
Triaging patient concerns.
Completing nursing assessments.
Collecting vital sign data.
Providing medication refills, lab orders, and administering injections as assigned.
Patient education.
Referrals management.
Obtaining records.
Maintaining patient charts and follow up needs.
Essential Functions- Kinship
Provides home and office-based community nursing services, including non-critical health assessments and examinations, information and referrals, health related advocacy, chronic disease self-management education and training, and health and wellness education and training on issues that impact kinship families, such as nutrition, physical activity, heart health/hypertension, disaster preparedness, stress management, infectious diseases, substance use, domestic violence, sexual and reproductive health, smoking cessation.
Develops, coordinates, conducts, and oversees 2 monthly on-going health workshops for clients, and community, including but not limited to the health and wellness issues listed above.
Maintains confidential health records in compliance with all agencies, funder, and licensing mandates.
Participates as a member of Health and Safety committee for San Mateo and Bayview locations.
Participates in weekly supervision with Nursing Director.
Participated in bi-weekly collaboration with the Director of Family Support.
Provides case management through home and office visits with families.
Reviews medical information for psychiatry referrals.
Demonstrates sensitivity to the culturally diverse presentation and needs of employees and clients and awareness of health disparities.
Effectively collaborates with Director of Family Support to ensure successful execution of program deliverables.
Applied Knowledge, Skills, and Abilities
Current California RN license.
Bachelor's degree in nursing.
Public Health nurse certificate preferred.
Bilingual English/Spanish preferred..
Knowledgeable of San Mateo and San Francisco County community health resources.
Demonstrates excellent nursing assessment skills necessary for developing on going care plans for clients and families.
Attuned to the current health needs of vulnerable and at-risk populations and groups, providing updated information based on research and evidence-based practices.
Able to work autonomously, maintaining schedule and communication with team.
Valid California Driver's License and insurance.
Current CPR certification.
Psychiatry experience preferred.
Minimum of 2 years of experience in community/public health.
Excellent verbal, written and communication and facilitation skills.
Salary Information
This position has a salary range of $49.86 an hour or $54.85 depending on years of clinical experience and bi-lingual language skills.
*Please note- the language differential is contingent on passing a language proficiency test.
