Definition

Under clinical direction of a Community Health Nurse and general supervision from the Clinical Services Manager, performs a variety of home health and care coordination functions needed in the continuum of healthcare for the Salt River Pima-Maricopa Community. The CHR does not provide acute or emergent visits for health evaluation. The community health services is 24/7 when needed and this position will be part of a work rotation for on-call assignments. This is a driving position. This job class is treated as FLSA Non-Exempt.

Examples of Tasks

Tasks:

Home Visits

  • Screen clients for appropriate services and follow a written plan of care when providing services.

  • Perform health screenings as part of a care plan which may include, but are not limited to, testing blood sugar levels, checking vital signs, home safety assessments, and foot screenings.

  • Make home visits to evaluate and monitor the health status, public health needs and general well-being of Community members and their family members of all ages residing in the Community.

  • Provide homemaker services as assigned.

  • Transfer, reposition, and assist with or provide personal care as needed within program standards.

  • Assist with medication management and self-administration.

  • Inspect homes and assist clients with resources to improve safety, promote health, and reduce injury.

Care Coordination

  • Coordinate activities and care between clients and the care team, including locating clients, delivering messages from providers, and reviewing instructions for self-care.

  • Collaborate with the care team to make referrals to other resource agencies, crisis services and other health and social services.

  • Assist clients during appointments for health related needs.

  • Help clients identify a support network to provide for day-to-day care.

  • Complete referrals as assigned.

Communication

  • Report evaluation findings to the care team.

  • Document and report activities accurately and in a timely manner, consistent with plan of care.

  • Participate in coordination of home health services with Registered Nurse, clinical and interdisciplinary teams.

  • Inform, educate, and empower patients and families/caregivers regarding health issues using evidence-based resources defined by standard operating procedures.

Physical requirements

  • Meet the physical demands of the position including moderately heavy physical exertion on a regular and recurring basis, i.e. assisting patients with substantial support to the individual in ambulation or transfer activities (wheelchair, to bed, to tub, to commode).

  • Drive for extensive periods of time in a safe manner, including the transport and accompaniment of patients to appointments as assigned.

  • Ability needed for vision includes close vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

  • Use hands and fingers to grasp and feel in the process of providing care.

  • Frequently stand, walk, reach with hands and arms, stoop, kneel, crouch, or crawl.

  • Sit occasionally.

  • Perform physically and mentally in a manner consistent with the duties assigned.

Professionalism

  • Complete CHR Basic Training within six months of hire date, and complete the Refresher courses every three years thereafter.

  • Complete the CNA to Caregiver bridge training program within six months of hire date.

  • Maintain safe and healthy working environment and uses safe work methods and procedures in accordance with program standards.

  • Maintain Community food handler's card.

  • Participate as a member of multi-disciplinary consultation and review teams for case assignments to develop strategies and referrals that support individuals and families/caregivers in achieving healthy state.

  • Update professional knowledge and proficiency in nursing through continuing education, staff meetings, training and conferences, etc.

  • Must retain Arizona Licensed Nursing Assistant in good standing.

  • Perform other job-related duties as assigned to maintain and enhance program mission and operation.

Knowledge, Skills, Abilities and Other Characteristics:

  • Knowledge of the culture, customs, traditions, history and government of the Salt River Pima- Maricopa Indian Community.

  • Knowledge of medical systems, behavioral health, population health, and social service resources available to Community members including, but not limited to, Indian Health Service (IHS), Medicare, Medicaid, ALTCS, SSI, etc.

  • Knowledge of evaluation, preventative and therapeutic procedures used in the Salt River Community health program.

  • Knowledge of federal, state and SRPMIC laws and regulations governing nursing assistant practice.

  • Knowledge of theory, principles and scope of practice for a licensed nursing assistant.

  • Knowledge of health systems, disease processes, and home health standards and regulations.

  • Knowledge of data collection and reporting.

  • Knowledge of health services and resources available to the Community.

  • Knowledge of universal and standard precautions while providing care to clients.

  • Knowledge of chronic disease management and prevention of complications.

  • Knowledge of the principles of growth and development for the appropriate age groups.

  • Skill to provide care relative to the client's specific needs.

  • Skill to perform Nursing Assistant functions.

  • Skill observing, evaluating and documenting the behavior and progress of patients.

  • Skill evaluating and determining client needs and communicating those needs and making referrals.

  • Skill in performing observation and techniques applicable to licensed nursing assistant procedures, as well as emergency medical procedures including First Aid and Basic Cardiac Life Support.

  • Skill maintaining accurate records and reporting information appropriately.

  • Skill in expressing compassion, empathy, tact, and patience toward peers, patients, and families/caregivers.

  • Skill with organization, multi-tasking, flexibility and creativity.

  • Ability to communicate in such a way as to maintain positive and effective working relationships with managers, peers, health care professionals, Tribal officials, representatives of resource agencies, patients and families/caregivers.

  • Ability to work independently as a productive member of a team.

  • Ability to organize and prioritize tasks.

  • Ability to maintain appropriate boundaries regarding professional relationships.

  • Ability to incorporate cultural differences, health beliefs and learning styles.

  • Ability to use electronic technology and software, including the electronic health record and Microsoft Office Suite at an intermediate to expert level.

Minimum Qualifications

Education: Graduation from an accredited Certified Nursing Assistant program is required.

Licensure:Licensed Nursing Assistant

Certification:Caregiver Certification preferred

Experience:

  • At least two (2) years of professional experience in a health-related field required.

  • At least one (1) year of experience working in home health preferred.

  • Experience working with Native Americans required.

Equivalency: Any equivalent combination of education and experience that will allow the applicant to satisfactorily perform the duties of the job may be considered when filling the position.

Underfill Eligibility:An enrolled Community Member whom closely qualifies for the minimum qualifications for a position may be considered for employment under SRPMIC Policy 2-19, Underfill

Special Requirements

  • Must be a member of a federally recognized tribe. Enrolled Salt River Community members will have preference.

  • Will be required to work outside normal work hours including nights, every other weekend and holidays in order to meet the 24/7 services provided as needed.

  • Will rotate on-call assignments.

  • Must have current Basic Life Support and Bloodborne Pathogens training prior to start of work. May complete the training through the Salt River Clinical Services Program before start date after completing background check.

  • Must possess and maintain a valid Arizona Driver's License.
