Overview

This position is a one year, grant funded position focused on oral health needs of the community.

Fairview Health Services is looking for a Community Health Outreach Assistant Manager for our corporate campus located in Saint Paul, MN. This is a part-time position that will work day hours during the work week, along with some Sundays. Provides leadership and program management for selected community health functions to improve the health status of the community. Leads, develops, and implements community programs and partnerships. Researches and analyses data regarding varying demographics, community health trends and behaviors locally and nationally to support the community health needs assessments as required by The Patient Protection and Affordable Care Act. Serves as a liaison between Fairview Health Services, its mission and vision to meet the needs of the community. Identifies, develops and implements Community programs and partnerships. Builds strong working relationship with partners such as Public Health, Public School districts etc.

Responsibilities/Job Description

  • Coordinates community health promotion initiatives

  • Represents Fairview to the community and extends offers of time and resources to strengthen community activities towards health promotions

  • Partners with community to provide services, leadership, and technical assistance.

  • Identifies opportunities, develops and participates in selected health promotion.

  • Manages community programs such as; bike helmet safety, health education school tours, sports first aid kits, middle school health fairs, mentoring programs, safety and first aid education, disease prevention and partnership activities with community health departments, schools, EMS, congregations, and other community groups.

  • Promotes networking efforts, partnerships, and collaborative activities.

  • Actively participates in internal and external committees and professional organizations that contribute to the role and personal development

  • Implements community programs and partnerships under direction of department manager

  • Links appropriate staff, services, and resources to identified community needs.

  • Develops plans based on age, social and cultural consideration, conditions, and other relevant needs.

  • Implement and coordinate activities necessary to execute program.

  • Evaluate program on an ongoing basis and make necessary adjustments based on evaluation.

  • Coordinate program activities and resources to fit program plan established.

Qualifications

Required Qualifications

  • Working toward Associate's or Bachelor's Degree, or someone who has achived either Degree
  • 3-5 years in community health field

Preferred Qualifications

  • Bachelor's or Master's degree in Community Health

  • Community Health worker certification

  • Dental experience or dental hygiene experience

  • Demonstrated expertise in identifying community need and creating and sustaining programming to respond to same

Other Required Qualifications

Strong organizational and interpersonal skills. Ability to manage multiple projects at one time, works independently with minimal supervision, and is able to make decisions and troubleshoot as necessary. Excellent computer skills with experience in Word and PowerPoint. Must be able to perform the physical demands of the job. Prefer experience in program management and/or community health. Strong ties to the community to assure relationships needed to be successful in the job

The Direct Support Professional (DSP) will train and assist individuals in Individualized Community Services so they can remain living as independently as possible in their communities. The Direct Support Professional (DSP) works directly with persons served, their communities, RNs, case managers, and co-workers taking into account the areas of services, designated hours, and support. The Direct Support Professional implements, trains and manages all programmatic outcomes for their caseload of Individualized Community Services participants. </p><p><br></p> <p><strong>Essential Responsibilities:</strong></p><ul><li>Be an advocate for people receiving services to live, work, and participate in their communities by raising concerns, initiating service development activities with individuals, and assisting individuals in becoming more involved in the decision-making process.</li></ul><ul> <li>Meet one on one with individuals in their homes and in community settings.</li> <li>Independently manage a caseload of individuals and hours authorized to 80% billable productivity rate.</li> <li>Assist individuals with reaching their personal goals, including but not limited to accessing community resources, expanding social and recreational goals, and building interpersonal and communication skills.</li> <li>Understand and comply with licensing requirements, including maintaining process notes and documentation as required to track progress towards establishing goals and outcomes.</li> <li>Participate in team meetings with people supported, as required.</li> <li>Provide direct support to individuals as assigned in your caseload.</li> <li>Participate in individual and group supervision with Program Manager.</li> <li>Serve as a resource to peers </li> <li>Serve as a representative of the agency to the people we serve, our community partners and external constituents. Serve as a positive role model for other employees.
