Job Description Job Description Job Summary: The Community Health Nurse is responsible for promoting and protecting the health of populations in the community through the delivery of preventative healthcare, education, and direct care services.
This role involves working closely with individuals, families, and groups to assess health needs, develop plans, and implement interventions aimed at improving health outcomes. The Community Health Nurse will collaborate with other healthcare providers, social services, and community organizations to enhance the well-being of the community. Key Responsibilities:
Health Assessments and Screenings: Conduct health assessments and screenings in community settings to identify health risks and needs. Collect and analyze health data to develop community health profiles.
Health Education and Promotion: Plan and implement health education programs and workshops on topics such as nutrition, physical activity, chronic disease management, and preventative care. Develop educational materials and resources tailored to the needs of the community.
Direct Patient Care: Provide nursing care and services, including immunizations, wound care, and management of chronic illnesses. Offer guidance and support to patients in managing their health conditions and accessing healthcare services.
Community Outreach and Engagement: Build and maintain relationships with community members, leaders, and organizations to foster trust and collaboration. Participate in community events and outreach activities to raise awareness about health issues and available services.
Case Management: Coordinate care for individuals and families, ensuring they receive appropriate services and support. Advocate for patients' needs and assist with navigating healthcare systems and resources.
Program Development and Evaluation: Assist in the development and implementation of community health programs and initiatives. Monitor and evaluate the effectiveness of programs and interventions, making adjustments as needed.
Documentation and Reporting: Maintain accurate and up-to-date patient records and documentation of services provided. Prepare reports on community health activities and outcomes for review by supervisors and stakeholders.
Professional Development: Stay current with best practices, emerging trends, and advancements in community health nursing. Participate in continuing education and training opportunities.
Qualifications: Education: Bachelor's degree in Nursing (BSN) required. Master's degree in Nursing, Public Health, or related field preferred. Licensure: Current Registered Nurse (RN) license in the state of New York Experience:
Minimum of 2 years of nursing experience, preferably in a community or public health setting. Skills: Strong assessment and clinical skills. Excellent communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team. Proficiency in using electronic health records and other relevant software. Certifications: Certification in Community Health Nursing or Public Health Nursing is a plus.
