To perform public/community health nursing services in clinics, homes and in the community; and to provide community and client centered counseling, education and information concerning illnesses.

This position will be located in the Immunization Clinic.

All Community Health Nurses and Case Managers are responsible for the following duties:

  • May provide health related counseling on the prevention of communicable diseases; coordinate health programs with outside organizations and agencies; introduce general health related information and instruction to individuals, families and community groups

  • May administer a variety of pre-examination tests including skin tests, blood tests, and other tests used in determining communicable disease; provide information and education to patients before and after tests as necessary

  • May provide health related information on the prevention of communicable diseases; coordinate health programs with outside organizations and agencies; provide general health related information and instruction to individuals, families, and community groups

  • Assess and evaluate patient's response to treatment and medication

  • Advise patients on available and appropriate medical and social services available

  • Keep accurate patient records; maintain confidentiality at all times

  • Stay abreast of new trends and innovations in the areas of disease prevention and treatment, chronic diseases

  • Compile and submit necessary medical/statistical reports and data in a timely fashion

  • Apply basic time management and patient care coordination skills to effectively provide nursing and health care services

  • Participate in quality improvement activities

  • Perform related duties and responsibilities as required

Nurses assigned to a Clinic have the following responsibilities:

  • May provide a variety of nursing services in varied Southern Nevada Health District Clinic areas by the use of the nursing process

Nurses assigned to Community Health and Case Managers have the following responsibilities:

  • Act as a resource and provide leadership to community groups and co-workers

  • Assist in research and investigation of communicable diseases when requested

  • Make home visits

  • May assess, plan, and implement care, evaluate patient's response to plan, make changes in plan as appropriate

Qualifications:
Community Health Nurse I

Knowledge of:

  • Principles, procedures, methods, and techniques of nursing practice

  • Medical terminology

  • Principles and procedures of medical record keeping

  • Pertinent Federal, State and local laws, codes, and regulations regarding health care

  • Universal precautions and infection control practices

  • Computer software and data bases utilized in the District's collection of data

Ability to:

  • Assess and evaluate patient's response to treatment and medication

  • Interview patients prior to examinations in order to assess their health status

  • Interview patients in order to determine their medical history, background and health status

  • Educate patients on a variety of health care related issues

  • Prepare, perform, and interpret laboratory tests

  • Provide responsible health and nursing care, including health education

  • Properly prepare and administer immunizations and vaccines

  • Administer a variety of pre-examination tests

  • Administer a variety of prescribed medications

  • Respond to emergency situations which may include a bioterrorism event

  • Administer first aid

  • Maintain confidentiality

  • Communicate clearly and concisely, both orally and in writing

  • Establish and maintain cooperative working relationships with those contacted in the course of work

  • Effectively learn and use computer programs designed for the Community Health Division

Training and Experience Guidelines

Training:

  • Equivalent to a bachelor's degree from an accredited college or university with major course work in nursing, or equivalent registered nurse experience. (The District will accept two years of registered nursing experience in lieu of a bachelor's degree.)

Experience:

  • One year of general registered nursing experience preferably within the last five years. The one year requirement may be waived when a Bachelor of Science in Nursing has been obtained within the last two years with coursework in Community Health Nursing; and a community health internship has been completed within the last two years.

Community Health Nurse II

In addition to the qualifications for Community Health Nurse I:

Knowledge of:

  • Basic management concepts, principles, and practices

Ability to:

  • Properly prepare and administer immunizations and vaccines

  • Provide administrative and nursing orientation to staff

  • Review the work of lower level nursing staff

  • Assess and evaluate patient's response to treatment and medication

  • Apply basic time management and patient care coordination skills to effectively provide nursing services and health care

  • Work independently in the absence of supervision

  • Demonstrate leadership skills

  • Effectively apply problem solving skills

Training and Experience Guidelines

Training:

  • Equivalent to a bachelor's degree from an accredited college or university with major course work in nursing or equivalent registered nursing experience

Experience:

  • Two years registered nursing experience including one year of community health nursing experience, preferably within the last five years

  • Community Health Nursing is defined as a nursing assignment in a community based program such as those in school nursing, jails/prisons, industrial settings, home visits, case management, or community outreach programs associated with hospitals. Private physicians' offices and hospital settings are not considered a part of community health unless there is an outreach aspect which the candidate has been assigned to for a minimum of one year.

License or Certificate:

  • Possession of an appropriate, valid State Registered Nurse license

  • Possession of, or ability to obtain, an appropriate, valid CPR certificate

  • Possession of, or ability to obtain, an appropriate, valid Nevada driver's license

Distinguishing Characteristics:Community Health Nurse I: This is the orientation class in the Community Health Nurse series. Employees in this class must receive appropriate orientation and exposure to the District's public health policies, procedures and protocols. Employees will be promoted to Community Health Nurse II after completing a probationary period and meeting the standards for the CHN II. Since this class is typically used as a training class, employees may have limited community, public health experience.

Community Health Nurse II: This is the full journey level class within the Community Health Nurse series. Employees within this class have a minimum of two years of community or public health experience and are fully aware of the District's operating procedures and policies and receive only occasional instruction or assistance as new or unusual situations arise. Positions in this class are flexibly staffed and are normally filled by advancement from the I level, or when filled from the outside, have prior experience.

Community Health Nurse II - Case Manager: This is a special assignment class within the Community Health Nurse series. The practice of community health field nursing shares many similarities with the practice of case management. Therefore, a Community Health Nurse II who is currently working in the field as a Community Health Nurse and who is accredited as a Nurse Case Manager by passing the National Nurse Case Manager examination may be advanced to the Case Manager position but only while working in a field position. If the Case Manager leaves the field position, the employee will return to the Community Health Nurse II classification and salary.
