Position Summary

Work with individuals or families in the community to provide public health services in the home, clinic, and community settings. This position involves communicable disease case investigation, immunization of vulnerable populations, education, prevention and treatment of diseases and services that promote health and prevent the spread of disease. Provide professional nursing services through home visits, clinic or community setting; acting as liaison between medical providers and patients; linking clients to community resources and services; prepare and review reports; coordinate program specific services in office or clinic setting; establish and report on goals; training on emergency preparedness; presenting health information to the public; communicating effectively with local health care providers and the public; investigating and interviewing individuals and families about public health conditions; maintaining confidentiality of patients.

Essential Functions

Work with individuals and/or families to address public health issues by conducting treatment, screening or education through home visits, clinic, or community services. Assuring adherence to state and federal guidelines and clinical practices.

Communicate with state, local and/or national partners regarding outbreaks or communicable diseases. Follow Bureau protocols and procedures for program specific reports and services. Conduct communicable disease investigations, contact tracing and reporting of communicable diseases, provide immunizations for uninsured, underinsured city residents. Coordinate clinic delivery of services as appropriate.

Establish and maintain relationships in the community by communicating with local health care providers regarding client diagnosis and/or treatment. Participate and collaborate with local coalitions/task forces to improve the community's health.

Represent the City in local, state or national meetings. Develop new or existing partnerships public health services within community. Present to community organizations on public health issues.

Coordinate program specific services by assuming leadership for program(s) services. Assure proper inventory of supplies, educational materials, and other items for efficient delivery of program services.

Develop and update policies and procedures for program services in cooperation with supervisors and other staff. Document and reporting program activities; participate in local initiatives related to program (s) to improve the community's health. Attending state and national meetings related to the program.

Coordinate educational messages for the public and individuals. Establish and maintain relationships and communications with local health care providers through phone, email, or fax. Order labs, x-rays, blood tests, etc.

Maintain continuing education credits related to program and/or public nursing in general. Utilizing computer programs relevant to program specific areas, such as PA National Electronic Data Surveillance System (NEDSS, PA State Immunization Information System (SIIS), Electronic Food Outbreak Registry System (EFORS), and electronic health record, CureMD.

Other duties as assigned.

Required Knowledge, Skills and Abilities

Work requires the occasional direction of helpers, assistants, seasonal employees, interns, or temporary employees.

Decisions regarding implementation of policies may be made. Contact may involve support of controversial positions or the negotiation of sensitive issues or important presentations. Contacts may involve stressful, negative interactions with the public requiring high levels of tact and the ability to respond to aggressive interpersonal interactions

This job title normally performs the job by following established standard operating procedures and/or policies. There is a choice of the appropriate procedure or policy to apply to duties. Performance reviewed periodically.

Work requires advanced skills and knowledge in approaches and systems, which affect the design and implementation of major programs and/or processes organization-wide. Independent judgment and decision-making abilities and necessary to apply technical skills effectively.

Ability to read literature, books, reviews, scientific or technical journals, abstracts, financial reports, and/or legal documents.

Ability to deal with system of real numbers; practical application of fractions, percentages, ratios/proportions and measurement.

Ability to write reports, prepare business letters, expositions, and summaries with proper format, punctuation, spelling, and grammar, using all parts of speech.

Minimum Acceptable Training and Experience

Registered Nurse licensed to work in the Commonwealth of Pennsylvania.

Over one (1) year up to including three (3) years.

Valid Class C Operators license issued by the Commonwealth of Pennsylvania.

Registered Nurse License issued by the Commonwealth of Pennsylvania. Licensed Practical Nurse (LPN) license.

All persons who become employed by the city on or after the effective date of this section (May 18, 2022) shall be or become residents of York County or any County contiguous of York County (Adams, Cumberland, Dauphin, Lancaster, Hartford, Baltimore, and Carroll Counties), within six months from the first date the employee achieves permanent employment status.

Drug screening testing with negative results.

Background report.

You must provide the following valid clearances:

PA Child Abuse History Clearance with "no record exists".

PA State Police Criminal Record History

Federal Bureau of Investigation (FBI) fingerprint -based Background check.

Mandated Reporter Certificate.
