Job Description:

The Community Health Educator (H11521) interacts with the Immunization Program team ; providers participating in immunization programs; as well as other partners and stakeholders (both internal and external) to achieve the goals and objectives of the program. Duties include conducting compliance and assessment site visits of participating providers, assisting providers with education and technical assistance regarding program requirements, updating policies and procedures of the program, and assisting with vaccine management among others.

Job duties assigned to this position include:

  • Keep informed on the most recent and up to date immunization information, recommendations, and licenses throughout the lifespan, and communicate that information to program staff, private providers, and public immunization clinics.

  • In designated region of the state, provide initial orientation, training of clinic staff, continued support, VFC and immunization updates to both public and private immunization providers. In addition, provide education on immunization updates, recommendations, NESIIS registry system, or program priorities to Vaccines for Children providers, nursing schools, school nurses and medical professionals.

  • Assist with vaccine management for the Immunization Program to include assisting with vaccine distribution, evaluating provider accountability, and management of expired and wasted vaccine.

  • Assist with VFC Compliance Visit Oversight, which includes conducting annual site visits with the VFC enrolled providers in the designated area and ensuring compliance with program requirements;

  • Travel both in and out of state as required.

Knowledge, Skills, and Abilities:

Knowledge of: the principles and practices of disease prevention within community health programs; the practices of health education; responsibilities of community health medical and nursing staff; the social characteristics and economic conditions which affect the health of individuals and families; grant and federal funding processes.

Ability to: learn and apply agency rules, policies and procedures; apply rules, policies and standards of Public Health to daily work; communicate individually and in groups with medical practitioners, health care staff, other governmental officials, and the public; establish and maintain working relationships; understand and apply supervisory and health care decisions based on nursing care principles and practices; collect health data and report on findings

Requirement / Qualifications:

Minimum Qualifications: Post high school coursework/training in community health education, biological science, adult/continuing education or related field AND experience in public/community health education activities with responsibility for: developing and evaluating health education programs; determining health needs; preparing educational and informational materials, training aids, and presentations; or conferring with individuals and groups to exchange information pertinent to community health education programs.

Other: Valid driver's license or the ability to provide independent authorized transportation. In state and out of state travel required.
