Summary Objective

The purpose of this position is to coordinate program activities and community outreach with other health, social service and community agencies.

This class works independently, under limited supervision, reporting major activities through periodic meetings.

ESSENTIAL FUNCTIONS

The essential functions listed below are those that represent the majority of the time spent working in this class. Management may assign additional functions related to the type of work of the class as necessary.

  • Assists in budget preparation and monitors and controls expenditures.
  • Collects statistical data and prepares reports regarding program activities.
  • Develops and maintains program records, policies and procedures.
  • Represents the department to the public and attends public meetings and conferences to discuss, inform, and instruct others concerning health services and programs.
  • Participates in quality assurance activities.
  • Schedules and coordinates public relations activities and public service announcements.
  • Coordinates program activities with other programs and community agencies.
  • Evaluates and recommends improvements in departmental procedures, programs and activities.
  • Plans and coordinates assigned public health program activities.
  • Monitors and ensures compliance to grant guidelines.
  • Conducts trainings, lectures and seminars on health or environmental issues.
  • Coordinates case management efforts relative to program assignment.
  • Performs related work as assigned.

SENIOR MEDICARE PATROL: COORDINATOR OF VOLUNTEERS

  • Studies trends related to volunteer management and implements ideas for program enhancement or expansion.
  • Provides leadership regarding recruiting, managing, and retaining volunteers.
  • Manages volunteers, provides support in meeting required standards.

SENIOR MEDICARE PATROL: COMPLEX INTERACTION MANAGER

  • Researches and manages program's complex interactions and when necessary, makes referrals to the appropriate entity.
  • Provides expertise on recent fraud trends and updates on the status of referred cases.
  • Adapts to the changing nature of reporting guidelines regarding suspected healthcare fraud, errors, or abuse.

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS

  • Bachelor's Degree in Community Health, Environmental Science, Health Care, Public Administration or related discipline.
  • Two (2) years of community, public health, case management, or social service experience or an equivalent combination of education, training and experience.

SENIOR MEDICARE PATROL: COORDINATOR OF VOLUNTEERS

  • Bachelor's Degree in Community Health, Environmental Science, Health Care, Public Administration, or related discipline.
  • Two (2) years of experience working with older adults, program planning, project management, program coordination, and program evaluation related to volunteers.

SENIOR MEDICARE PATROL: COMPLEX INTERACTION MANAGER

  • Bachelor's Degree in Community Health, Environmental Science, Health Care, Public Administration, or related discipline.
  • Two (2) years of experience working with older adults or courses in gerontology.

OTHER MINIMUM REQUIREMENTS

  • Must possess and maintain a valid state driver's license with an acceptable driving history.

PHYSICAL DEMANDS

Physical demands refer to the requirements for physical exertion and coordination of limb and body movement.

  • Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.

UNAVOIDABLE HAZARDS (WORK ENVIRONMENT)

Unavoidable hazards refer to the job conditions that may lead to injury or health hazards even though precautions have been taken.
