Note: This is an in-office based position assisting Care Coordination in the Albuquerque area and surrounding counties, with visits to members' homes, hospitals, and community partners.

Job Purpose:
This position is responsible for contacting members and community partners in assigned areas and is responsible for performing ongoing functions to support the Social Service department. Responsible for contacting identified members to inform and educate them on health care programs, responding to inquiries from members, and supporting the clinicians in the Physical and Behavioral Health Management department with their provider and member activities. This position is also responsible for developing relationships with community partners, working with community leaders and contributing to the design of community health outreach programs. Also assists as needed in identifying local social service resources for members.

Required Job Qualifications:

  • Bachelor Degree OR 4 years health, social services or community advocacy experience,

  • Knowledge of medical terminology

  • Experience coordinating member related needs, providing assistance to members, and analyzing member needs

  • PC proficiency including Microsoft Office applications

  • Customer service skills

  • Verbal and written communications skills including developing written correspondence to members and to other department personnel and coaching skills, including motivational interviewing, to educate members on medical and BH issues

Preferred Job Qualifications:

  • Bachelor's degree in Social Work

  • Community Health Worker (CHW) certification.

  • CA

  • CB

Primary duties may include, but are not limited to: * Defines the health outcomes environment through development of key stakeholder criteria, identification of desirable community partners, availability of local programs and opportunities. * Provides/promotes provider training to assure compliance with government mandated health education programs and the health education referral system. * Establishes positive relationships with community based organizations and other key partners to gain understanding of the cultural and linguistic needs of members. * Recommends health education programs based on analysis of State/Regional/Local health statistics, disease prevalence, census data, and health care service usage. * Maps community based assets and collaborates with key stakeholders to leverage available resources for development of new and accessibility to existing health education/promotion programs. * Develops and implements strategies that promote local health education resources and address identified member health education needs Qualifications: * Requires a BSN; 4 years of clinical experience in health care /managed care and with data analysis and interpretation; or any combination of education and experience, which would provide an equivalent background. * Current unrestricted RN license in applicable state(s) * Must have ability to comprehend medical policy and criteria to clearly articulate health information. * Local Travel required. * Medicare and Medicaid knowledge- highly preferred * Able to coordinate outreach to members in the community to attend local events and maintain a partnership; member facing; cultural alignment * Network/Identify with existing vendors, Expos to participate and align Anthem with scheduled events * Must be able to analyze data to better understand the population care gaps to get members to the health events/fairs * Experience as a community health worker explaining wellness prevention to the population * 3 years of HEDIS experience highly preferred * Background in Community/Public Health experience highly preferred * Spanish bilingual preferred.
