General Description
The Community Health Associate II is a valuable member of our Women's Health Program who under direct supervision, assists in preparing for and offering direct individual and community services, client advocacy, and providing both general and specific health interventions in the community and clinic settings. Work assignments are in-depth and specialized, focusing on specific areas of responsibility and independence of action.
To be successful in this role, we are looking for someone who:
Fully represents the Women's Health Program to other sections within Lake County government, the community, and other organizations in the most positive and responsive manner.
Maintains an exceptionally high level of patient confidentiality.
Maintains a collaborative and friendly working relationship with all other LCHD/CHC programs
Schedule: 37.5 hours per week
Essential Functions
Provide direct services to clients in community and clinic settings according to need and established program protocols/procedures.
Implement activities for clients.
Assess learning needs of specific clients and adjust direct services to meet those needs
Engage client in program services according to established protocols and procedures.
Assist in marketing services and recruiting clients into needed services, includes community outreach.
Develop and maintain positive contacts with allied community service providers.
Provide follow-up and referral to appropriate allied community services.
Maintain appropriate documentation of direct services in accordance with established program procedures.
Maintains accountability for work through timely submission of quantitative and qualitative data direct services.
Participate in staff meetings and assigned training activities.
Provide input into development of annual program goals and offers feedback/information necessary to assess progress toward their achievement including participation in grant site reviews.
Actively contribute to program evaluation activities.
Develop and maintains computer skills needed to complete assigned duties.
Develops and maintains a working knowledge of individual Emergency Management roles
Participate in reproductive health education presentations to clients and community groups
Knowledge Skills Abilities and Education Required
Associate's degree, high school diploma or GED.
With associate's degree, two years of relevant work experience.
With high school diploma, four years of relevant work experience.
