To provide proper supervision and help ensure the safety and well-being of the individual at all times.
To have the individuals best interest in mind and focus on a person centered approach.
Focus on valued outcomes that the individual would like to achieve to help improve the quality of life.
Assist members with activities of daily living.
Access the community with the individual as needed.
Attend all staff meeting and training sessions.
Keep the supervisory staff informed of all issues related to the service.
Report and document all incidents that may occur with the individual within 24 hours.
Implement and follow the authorized plan.
Complete accurate records and data of the individuals progress in accordance with the frequency noted.
Submit all documentation on time and in an appropriate fashion.
All other duties as assigned by supervisor.
Must attend 3 day mandatory New Hire Training (Mon-Wed from 9am to 5pm)
HS Diploma/GED, some experience working with people with Developmental and Intellectual disabilities
