ChristianaCare is currently seeking a Community Educator for the Child Development Watch program at our Riverside campus. In this position the selected individual will promote access to services, provide health education and assist patients in navigating the health care and social services systems. Other responsibilities to include:
Develop and deliver health education for Child Development Watch patients and families.
Create connections between vulnerable populations, the health care system and community/state resources.
Assist patients at ChristianaCare Child Development Watch.
Help patients identify issues affecting their overall health and develop health/social management plans and goals.
Facilitate healthcare and social service system navigation.
Educate staff, health providers and stakeholders about patient needs.
Provide culturally appropriate health education on topics related to chronic disease prevention.
Advocate for underserved individuals to receive appropriate services.
Requirements:Associates degree. Bachelor's degree preferred. One-year community and patient experience preferred. Computer experience required. Demonstrated ability to work with diverse populations required. Bi-lingual language skills are preferred.
This is a Part-time position working 48 hrs bi-weekly, Day shift.
