PURPOSE

The Community Health Educator (CHE is responsible for leading the Company's community outreach efforts through the execution of oral health education to improve the overall health, well being and smiles in their communities. The CHE serves as a liaison between families in need and community, social service, educational and health service providers while delivering the company's vision, mission and values throughout the community.

ESSENTIAL JOB FUNCTIONS

  • Educate the community on the need for oral healthcare through the implementation of the core areas:

  • Referrals (25%)

  • Presentations (35%)

  • Screenings (5%)

  • Community Events (10%)

  • Community Agency Alliance (25%)

  • Office time to schedule daily leads

  • Develop and execute monthly outreach plans

  • Promote awareness of oral health issues by the developing and cultivating relationships with community agencies, school systems, healthcare disciplines, non profit organizations and Chambers of Commerce

  • Serve as a liaison between needy families and oral health service providers;

  • Develop and cultivate relationships with key community leaders as well as appointed and elected officials

  • Work with staff in Kool Smiles offices to encourage community involvement

  • Assist in the execution of Public Relations activities, including grand openings and family fun days at the office

Assumes additional responsibilities and performs special projects as needed or directed

REQUIRED QUALIFICATIONS

Education, Training and/or Experience

  • Graduation from high school is required, some college is preferred but not required.

  • Experience and comfort in public speaking

  • Experience in cold calling

  • Excellent written and verbal communication skills

  • Computer Skills -- Microsoft Office (Word, Excel, Powerpoint)

  • Familiarity with Medicaid system and its population is a bonus

PHYSICAL DEMANDS

Nature of work requires an ability to effectively communicate and exchange information with patients, their parents, and staff. Must be able to walk and stand for extended periods of time; ability to lift/carry up to 30 pounds and be able to twist, turn, bend and stoop.

WORKING CONDITIONS

Majority of work performed in the community. Weekly travel required to off-site locations in completing work requirements, attending meetings or events.Requires some nights and weekend availability for extended hours during peak periods. Hours may change to meet business needs.

Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community. Today, the CSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. Norfolk CSB seeks team members interested in providing skilled services in a public service environment. We are also seeking persons to join our team that are dedicated to supporting a positive work and treatment environment that instills hope and recovery opportunities for the persons we serve. The CSB is seeking to hire an Early Childhood Special Educator to become a member of the inter-disciplinary team. Essential Functions Essential functions include but are not limited to: * Conducts developmental assessments and works with infants in the following activities in the home environment and in small groups at various Norfolk sites: cognitive skills, social-emotional skills, language skills, gross and fine motor skills, and adaptive skills. * Has working knowledge of various screening and assessment tools that are used with children with disabilities age birth to three. * Develops and implements objectives outlined in the Individual Family Service Plan (IFSP) following the initial assessment. * Documents developmentally appropriate instructional objectives and task analysis programming for young children with handicapping conditions. * Maintains intensive child and family records and documentation in accordance with federal and other applicable guidelines. * Reviews and updates child and family's goals and objectives as established in the IFSP on a regular basis. * Schedules and provides educational activities in the home environment or other natural environment as identified by the family, and in compliance with the IFSP. * Coaches parents in various intervention strategies and works to engage the parent in active participation in home-based activities. Instructs and encourages parents to carry out IFSP objectives in the home and provide parents with information about child growth and development. * Performs other related work as assigned. Education/Experience Bachelor's degree in special education or related educational field. Experience working with infants and toddlers or children with developmental delays. Additional Information & Requirements Work schedule: Monday - Friday, 40 hours completed sometime during the hours of 8:00 a.m. - 6:30 p.m. Valid Driver's License required.
