Charlotte Hungerford Hospital is a 122-bed, general acute care community hospital located in Torrington, Connecticut, that serves as a regional health care resource for the 100,000 residents of Litchfield County and Northwest Connecticut. The Community Case Manager provides direct and collateral support to individuals with opiate use and/or other substance use disorders - recovery or currently using substances - to facilitate client access/participation in all community supports needed to obtain and sustain recovery. The Community Case Manager also supports clients' success in recovery by providing crisis support and assisting with tasks related to achieving Stages of Change objectives. This position works on behalf of all formal and informal community providers to unify community support behind individuals seeking recovery from opiate use and/or any substance use disorders.

The Recovery Case Manager will work closely with first responders to engage individuals with an opiate and other substance use disorder who had interactions with law enforcement and/or EMS personnel because of substance use or an overdose.
The goal is to engage those individuals struggling with addiction who have had a medical intervention or contact with law enforcement and offer a pathway to recovery
Embracing a 'Stages of Change' model, The Recovery Case Manager will provide case management support and engagement while individuals are considering entering recovery. The Recovery Case Manager will connect clients to the program and services they need when they make the choice to begin recovery which may include all case management support such as: transportation, basic needs, etc.
The Recovery Case Manager will work and engage with clients out in the communities of NW Connecticut including the rural NW Corner. Community work will be mobile and take place in private homes, social service agencies, churches, court, police departments and other non-traditional settings.
The Recovery Case Manager will work closely with law enforcement and EMS personnel in their workplace settings. This will include joining law enforcement on patrols to learn about the community and work they do while responding when appropriate to live events in the community.
The Recovery Case Manager will need to work flexible hours with a focus on afternoon and evening engagements. The ability to be on call and respond on weekends will be needed. The Recovery Case Manager must be available during evening/night hours and weekends. The need to travel throughout Litchfield County will be necessary.

Past professional experience in the field of substance abuse and recovery required.
Individuals with first responder, military and/or other life experiences unique for this role are sought.
Bachelor's Degree with extensive life experience in the recovery field required.
Master's Degree in Human Services, Social Work, or related field preferred
High flexibility, ability to work independently, and willingness to work evening hours required
Excellent communication skills and ability to engage well with first responders/law enforcement and the public
Personal experience with recovery from a substance use disorder preferred but not required
