Community Care Worker IV

Serves as an advanced worker in providing direct delivery and contract services to Long Term Care Services individuals. Duties include establishing eligibility for social services, completing needs assessment, and making referrals to contract or community agencies. Reviews and responds to legislative and non-legislative inquiries. Additional duties may include providing assistance and policy interpretation for other staff, training new intake screeners, or completing special reports. Work includes operating a computer. Will work on special projects for the region to include all data entry involving contract terminations and change of ownership as notified by our Contracts Division. Document's information manually and into multiple automated systems such as SASO, CSIL, SharePoint, NTK. Verifies, troubleshoots, and/or research case information utilizing multiple automated systems such as SASO, TIERS, SharePoint, CSIL, NTK. Assists the Intake Supervisor with supervision of unit and CMS Coordinator with contract assignments as needed. Completes monthly reports as directed by Intake Supervisor. Maintains confidentiality of all incoming and outgoing information. Provides outstanding customer service in person, over the telephone and via e-mail. Maintains a positive and productive attitude while working in a fast-paced environment. Monitors and prioritizes work to ensure timely and appropriate program services are delivered, transferred, or terminated. Must be willing to serve as a floater if needed to assist units with home visits when needed. Essential Job Functions:

  • Determines eligibility for services according to program policy and procedures.

  • Develops and implements service plans for clients.

  • Provides ongoing case management to monitor individuals' condition and assesses services provided in resolving other problems.

  • Maintains case records to form a written record for each individual.

  • Prepares reports and records to provide management information or program operations.

  • Provides administration assistance to the supervisor which may include serving as acting supervisor, planning and conducting staff meetings, assigning work, counseling, monitoring staff performance. and monitoring caseloads.

  • Operates a computer to document, update, and inquire into case records.

  • Interprets policies to staff.

  • Mentors staff.

  • Assists in training new staff.

  • Assists the unit supervisor in completing reports or resolving special case problems.

  • Serves as a resource person for more difficult or complex cases.

  • May recruit and supervise volunteers.

  • May read cases for the unit.

  • Performs related work as assigned Knowledge Skills Abilities:

  • Knowledge of gerontology, the dynamics of the aging process, and special problems of Long-Term Care Services individuals.

  • Knowledge of interviewing techniques to obtain personal information, make inquiries and resolve conflicting statements.

  • Knowledge of the English language such as vocabulary, grammar and organization of expression, composing correspondence, and reporting casework.

  • Ability to listen to individuals, exploring and reflecting feelings, and present relevant alternatives describing fees, bills and consequences of choices.

  • Ability to complete work within required timeframes.

  • Ability to appraise living conditions, including physical and emotional environment.

  • Ability to establish priorities according to relative importance and deadlines, set goals and manage time efficiently.

  • Ability to monitor unit operations.

  • Ability to observe employee performance and record observations.

  • Ability to communicate orally and in writing.

  • Ability to establish rapport with individuals.

  • Ability to communicate with community members to request assistance for individuals, promote support for the individual or coordinate resources and programs.

  • Ability to read cases effectively and accurately.

  • Ability to communicate policies and procedures to staff. Registration or Licensure Requirements:

  • Valid Driver's License Initial Selection Criteria:

  • Must have at least two years' experience as an Intake Screener II with Community Care Services

  • Professional experience working in social services.

  • Experience with the use of computer software such as SASO, NTK, TIERS, CSIL, Scheduler, Word, Excel, SharePoint, Internet, Outlook and CAPPS.

  • Experience which demonstrates knowledge of the English language such as vocabulary, grammar and organization, composing correspondence as demonstrated on the application.

  • Prefer at least one year of mentoring experience.

  • Prefer experience serving as acting supervisor in the unit supervisor's absence.

  • Application indicates willingness to travel at least 50% of the time. Additional Information:

  • This position is open only to current or past CCSE Intake Staff in Region 11*

  • Must have reliable transportation.

  • Must be able to conduct home/site visits in assigned geographical areas which may involve exposure to substandard and/or unsanitary living conditions.

  • Must be able to attend training which may require overnight stay.

  • Must be willing to travel up to 50% of the time.

  • Occasional overtime may be required.
