The Community Care Coordinator (CCC) is involved in assisting with outreach and coordination with all Permian Basin services that are Juvenile Justice Stakeholders. The CCC assists the department in monitoring the provision and quality of services provided to the family and is the family liaison when new services/resources need to be sought or developed. This staff serves as the representative for the department for referrals and referral processes to community agencies that are providing preventative/intervention/rehabilitative services or any type of support for juveniles of Midland County. The CCC is a member of the program team that works to identify community resources available to meet current department needs for prevention/intervention/rehabilitation.

  • Outreach and coordination with all community agencies in Midland County that provide services and/or support to juveniles in the area of prevention/intervention/rehabilitation.

  • Assist JPO's with referrals to community agencies for preventative/intervention/rehabilitative services or any type of support for juveniles.

  • Works in collaboration with Programs, JPO's and outside community agencies to assist in securing services to meet identified needs and brokers both traditional and non-traditional services individualized to meet family needs.

  • Assists to identify service gaps and/or lack of community resources.

  • Serves as department representative to the Community Resource Coordination Group (CRCG) meetings.

  • Knowledge of Wraparound Services principles and processes.

  • Serves as JPD Liaison in the community to identify resources locally.

  • Serves as department point of contact for community program/service providers.

  • Facilitates all JMT (Juvenile Medicaid Tracker) referrals from the department.

  • Provide recommendations to Deputy CJPO Programs regarding community services for juveniles.

  • Provide support, guidance, and advocacy for youth, when appropriate, in the school setting, legal setting, and in the community.

  • Assist with the development and implementation of curriculum-based programs for youth.

  • Maintain regular contact with families; model and reinforce appropriate social interactions for them.

  • Assist JPO's with field visits and placement visits.

  • Assist JPO's with transport for youth to appointments and any services in/out of Midland County.

  • Willing and able to work non-traditional hours when needed.

  • Perform all other related duties and responsibilities as assigned for the purpose of ensuring the efficient and effective functioning of the department.

EDUCATION and/or EXPERIENCE

Must be over the age of 21, a U.S. Citizen, have valid Texas driver's license, pass a criminal background check.

Requires bachelor's degree plus at least one year of experience working with juveniles, or one year of graduate study or any equivalent combination of education and experience that provides the required knowledge, skills, and abilities

OR High School Diploma and five years of experience of wraparound principles/processes, family partnering and/or youth advocacy.KNOWLEDGE OF

Juvenile justice system; Texas Family Code, juvenile law; all TJJD standards, and standard office practices and procedures.

SKILL/ABILITY TO

Operate computers; read & write reports, letters, and other documents using proper format and grammar. Communicate effectively, both orally and in writing; physically restrain other individuals; and establish and maintain effective working relationships with co-workers, juveniles and their families, and the general public.
