Program OverviewCHRIS 180-Fulton County services are rooted in trauma informed care. This care is directed by an understanding of neurological, biological, psychological and social effects of trauma. We assess consumers to learn if trauma occurred and whether it has a role in behavioral health needs. Some trauma includes: sexual, physical, emotional abuse, severe neglect, loss, witnessing violence, substance abuse, imprisoned family member, and abandonment. Understanding when trauma occurred helps us tailor our responses, treatment and services. Trauma-informed assessments and interventions always acknowledge, respects, and integrates cultural values, beliefs, and practices.Position DescriptionThis position consists of rehabilitative, environmental support and resources coordination considered essential to assist a consumer/family in gaining access to necessary services and in creating environments that promote resiliency and support the emotional and functional growth and development of the consumer/family.The primary duties include but are not limited to the following: * Provide positive role-modeling to consumers/families;* Promote self-esteem * Promote healthy boundaries * Promote good hygiene and appropriate personal appearance among youth and young adults * Reinforce socially acceptable behaviors (ex: manners, eating habits, safety)* Teach and reinforce thoughtful decision making skills * Support in making positive change through motivational interviewing * Support the change process through a comprehensive understanding of the Seven Challenges program * Coordinating Services and Collaboration;* Make necessary referrals to meet consumer's health, well-being, education, mental health, and employment needs (i.e. Department of Public Health, Department of Family & Children Services, Georgia Department of Labor, employment staffing agencies, local colleges and tech schools, etc.)* Participate in Team Meetings for all consumers/members/residents/young adults * Collaborate and communicate with other programs in the CHRIS 180 continuum in the provision of services to consumers * Collaborate and consult with other providers and entities in the provision of best care services to consumers * Attend monthly RBWO meeting or provide progress in areas for a counseling summary for group home consumers * Attend monthly Child & Family Team Meetings (CFTM) or provide progress in areas for a treatment summary for consumers receiving wrap services * Advocate on behalf of consumers with other agencies to include coordinating services *
  • Monitor the provision of services provided * Specific services to consumers;* Contact must be made with consumer receiving Community Support services a minimum of twice each month. At least one of these contacts must be face-to-face and the second may be either face-to-face or telephone contact depending on the consumers support needs and documented preferences of the family * At least 50% service units must be delivered face-to-face with the identified consumer receiving the service * At least 80% of all face-to-face service units must be delivered in non-clinic settings over the authorization period * Demonstrate exceptional engagement skills in providing trauma-focused mental health treatment to target consumer population * Provide evidence-based trauma-informed individual treatment to consumers of all ages who meet criteria for trauma services * Provide skill building services to clients at North and TREE* Translate sessions for other staff working with the clients * Manage caseload. Number of consumers on caseload is based on frequency of sessions * Identification, with the consumer, of strengths which may aid in achieving resilience, as well as barriers that impede the development of skills necessary for age-appropriate functioning in school, with peers, and with family * Support to facilitate enhanced natural and age-appropriate supports * Coach caregivers in supporting consumer through understanding the stages of change * Assistance in the development of interpersonal, community coping and functional skills (including adaptation to home, school and healthy social environments)* Encouraging the development and eventual succession of natural supports in living, learning, working, other social environments * Supervise/observe the consumer (BA Services) or when instructed to do so in accordance with the IRRP* Assistance in getting to and from designated activities (BA Services) in accordance with the IRRP* Assistance in the acquisition of skills for the consumer to self-recognize emotional triggers and to self-manage behaviors related to identified stressors or symptoms * Assistance with personal development, school performance, work performance, and functioning in social and family environment through teaching skills/strategies to ameliorate the effect of behavioral health symptoms; * Assistance in enhancing social and coping skills that ameliorate life stresses resulting from the stressors or behavioral symptoms * Assistance to consumer and other supporting natural resources with illness understanding and self-management; * Assist the consumer/family in managing or preventing crisis situations; * Use preventive intervention techniques when needed to prevent injury (to self or others)* Service and resource coordination to assist the consumer and family in gaining access to necessary rehabilitative, medical, social and other services and supports * Must be willing to work in the community neighborhoods, the home, school, and public housing communities * Must be flexible in providing additional services for programs/contracts/grants (MAAC, PSSF, CCFA, SOC, Wraparound, etc)* Documentation Requirements;* Timely documentation of all case activities in accordance with Center policy * Maintain detailed files and other documentation in 100% compliance with standards * Complete all billing expectations within designated timeframes * Practice Requirements;* Adhere to the organization policies and procedures * Attend group supervision at a minimum of monthly * Attend Fellowship meetings * Participate in clinical consultation as needed * Review case studies * Provide feedback for hard to manage cases * Performance and Conduct;* Maintain confidentiality of consumer information * Maintain appropriate professional boundaries (written, verbal, physical)* Follow CHRIS 180 dress code * Follow the CHRIS 180 Code of Ethics * Follow mandated reporting requirements of GA Code * Support and maintain an environment of emotional and physical safety for consumers, staff and other stakeholders at all times * Demonstrate customer service;* Promote positive image of CHRIS 180 programs at all times * Work as a contributing and collaborative team member * Respond to consumer, referral and peer needs in a timely fashion * Answer telephone calls and emails accordingly * Respond to requests for information from stakeholders in a timely fashion * Maintain Personal Information and Training;* Attend and participate in staff meetings as scheduled * Attend and participate in required trainings as scheduled * Maintain required certifications * Review, understand and responsible to program policies * Provide documents in a timely fashion to Human Resources * Report changes in status to HR as required by policyQUALIFICATIONS/COMPETENCIES* High school diploma or higher degree behavioral health field or related field experience preferred, * An understanding of the concepts of child care, * Skilled in assisting and maintaining consumers in their family homes and/or community, * Ability to effectively relate to consumers, parents/custodians of all social, economic, ethnic and educational backgrounds, with firmness, fairness and concern,* Solid decision-making ability as related to the safety and well-being of consumers, families, community and staff,* Be guided by precedent and established policy and procedure in decision-making. This person will be required to analyze and react to difficult situations, often while under considerable stress and pressure, * Dependability, emotional stability and physically capable. This position requires some physical functions that are essential to satisfactory performance of the outlined job responsibilities, i.e. sitting, standing, walking, driving, speaking, hearing and the ability to physically restrain (when necessary),* Ability to recognize when assistance and consultation is needed. * Ability and/or experience in working with severely emotionally disturbed children, adolescents and their families Must be free to travel as the position demands,* A valid drivers license in the State of Georgia * Must be at least twenty-one (21) years of age
