Purpose Statement

The Community-Based Therapist/Case Manager provides Case Management services, In-Home Therapy services, and Group Skills services to separate caseloads of clients. Case management services assist an identified client with severe emotional disturbance and their family connect to required community support services. These might include mental health, social, educational, health, vocational, recreational services, in addition to related services such as transportation and legal services. Case management includes assistance in obtaining a comprehensive diagnostic assessment (if not already obtained), ongoing functional assessments, ongoing goal plans, and coordination of care with other agencies. In-Home Therapy in this role includes individual and family therapy work using a variety of modalities to help clients identify and make progress toward mental health goals. Group Therapy in this role includes children's therapeutic and skills groups facilitating mutual support, therapeutic processing, and development of social, communication, and cognitive behavioral skills. Essential Functions

  • Carry an average minimum of active Case Management clients, In-Home Therapy clients, and group clients appropriate to track

  • Develop effective relationships with clients and their families

  • Use culturally competent, evidence-based practices, reflecting the integrity of Youable's strengths-based perspective.

  • Complete an average minimum of 2 Diagnostic Assessments per month.

  • Ensure that all documentation including client health records, payer authorizations, and outcome reporting is done within the required time frame and in accordance with organizational, DHS, insurance, and county guidelines.

  • Organize and manage a personal schedule of home and community visits related to synchronous agency meetings.

  • Participate in both initial and ongoing training necessary to support current best practices.

  • Attend consultation, team, and other meetings as directed by the supervisor or by organization policy.

  • Follow and enforce all organization policies and procedures.

  • Evidence outcomes that show risk factors have decreased and growth has occurred or is in progress.

Qualifications

Education and Experience

  • Master's degree in psychology, marriage and family therapy, social work, or related field with appropriate licensure as required.

  • Experience completing diagnostic assessments and treatment plans preferred.

Skills and Abilities

  • Demonstrated organizational and time management skills.

  • Demonstrated verbal and written communication skills.

  • Demonstrated proficiency in a variety of basic computer programs and data entry systems.

  • General knowledge and understanding of case management services.

  • Awareness of evidence-based therapeutic modalities and mental health resources.

  • Capacity for cultural humility in interacting with people unlike yourself.

  • Demonstrated ability to work independently, identify strengths in individuals and systems, and positively motivate others.

  • Demonstrated ability to identify and resolve problems independently with both clients and professionals within the community.

  • Adaptability and creative problem solving for work in a fast-paced, sometimes stressful environment that may include crisis management.

Physical Demands/Environmental Factors

  • Regular local travel throughout the metro area.

  • Must have a valid driver's license, working vehicle, and auto insurance.

  • Visits and services provided in various environments, including in-home, in the community, and schools.

  • Availability to work up to two evenings per week.

  • Sitting for extended periods of time.

  • Ability to lift up to 25 pounds.
