Position Summary

This position is responsible for the provision of case management services to tenants of 17 units of housing in the community funded through the Center for Housing and Health. Responsibilities include: intake, intensive outreach and engagement, needs assessment, service planning, linkage to community resources, crisis intervention, documentation, evaluation, follow-up, evaluation and advocacy. This position works primarily in the field.

Essential Duties and Responsibilities

Case Management

  • Provide case management services including intakes, assessments, service planning, documentation, referral and linkage management, crisis intervention, evaluation, follow-up and advocacy to a caseload of 17 women who are formerly homeless.

  • Recruit landlords and assist landlords in the subsidy approval process. Act as a liaison between landlords, tenants and the Center for Housing and Health in relation to rental subsidies, leases and lease compliance.

  • Plan and implement groups and activities when appropriate.

  • Respond quickly and appropriately to crises utilizing established guidelines

  • Work to build relationships for effective service provision through regular active engagement with participants. This will include off-site home visits with program participants.

  • Act as a member of the Clinical Services team of Deborah's Place by collaborating with other case managers, attending group supervision and large clinical meetings, and assisting with Clinical Services activities.

  • Assist in coordinating tenant selection and intake process, including screening interviews, intake interviews and needs assessments.

  • Work with the landlords to coordinate tenancy and to resolve landlord/tenant disputes. Assist tenants in meeting all lease requirements including documentation and income certification. Work with tenants to assist them in addressing issues in order to prevent eviction. Make appropriate referrals for services based on participant goals and needs and ensure that linkages are secure and functional.

  • Provide education and resources based on the needs of participants with regard to maintaining housing and improving income status, community involvement, personal hygiene, life skills, and other identified needs.

  • Maintain regular contact with off-site linkages regarding participant concerns and act as a representative of Deborah's Place to external resources.

  • Maintain current and orderly individual participant files, including progress notes, intake and needs assessment, service plans, documentation of referrals and releases of information

  • Apply the Deborah's Place service model in individualized work with tenants

Center for Housing and Health Project Team

  • Participate in Center for Housing and Health project team meetings and training programs.

  • Collect data and provide reports as required by Center for Housing and Health and for Deborah's Place. Enter all data into ClientTrack system in a timely manner.

Qualifications and Requirements

Education: Bachelor's degree required; Master's degree preferred

Licensure and/or Certification: N/A

Job Experience: 2-3 years of case management experience.

Special Knowledge &

Knowledge of issues surrounding homelessness, mental illness and substance use; proficiency in Microsoft office software, good oral and written communication skills, good organizational skills, conflict management and crisis intervention skills; demonstrate clear and professional boundaries; act in a professional manner; Valid Illinois driver's license, proof of insurance and must have own vehicle.

The in-home setting could be a patient's private residence or in a patient's room in a SNF/NF, in an assisted living facility, or in an independent living facility. Vitality to You allows the Physical Therapy Assistant to evaluate and treat in the most functional environment which is the patient's home. The Physical Therapist Assistant provides physical therapy treatments to patients to facilitate increased independence and functioning under the supervision of a Licensed Physical Therapist. RESPONSIBILITIES/ACCOUNTABILITIES: 1. Attends and contributes to patient care, staffing conferences and other required meetings as a representative of physical therapy, under the direction of the physical therapist. 2. Responsible for adherence to department and patient treatment schedule, timely clinical and billing documentation requirements as well as work related meetings. 3. Works with other members of the rehabilitation team to implement programs and activities consistent with the needs and capabilities of each patient. 4. Acts as an assistant to the Physical Therapist when the Physical Therapist is performing tests, evaluations, and complex procedures. 5. Instructs patients, families and other caregivers in the skills and techniques of the physical therapy treatment program under the supervision of the Physical Therapist. 6. Maintains appropriate and timely documentation for all patients treated. 7. Cleans, maintains, identifies, and reports the need for repair of equipment, as necessary. 8. Puts Patient Service First: Ensures that patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights. 9. Implements patient's individualized treatment plan as established by the primary Physical Therapist. 10. Provides individualized physical therapy treatments including but not limited to: * Therapeutic exercise including aerobic capacity conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training * Functional training * Manual therapy * Airway clearance techniques * Integumentary repair and protection * Use of electrotherapeutic, physical agent and mechanical modalities * Health and Wellness * Falls Risk Interventions * Chronic Disease Management Education 11.
