The Community Assessor works locally in collaboration with a Community Engagement Specialist and other professionals to support family caregivers who are caring for a loved one at home. The Community Assessor visits families at home to conduct a family-centered assessment, determine a caregiver's needs, and intake eligible caregivers to the Structured Family Caregiving program. The Community Assessor partners with a centralized caregiver coaching team to ensure timely access to services. Position concentrates in the Rapid City area of the state.
What You Will Do:
Conduct home visits and complete electronic documentation on a hand-held device using Seniorlink's proprietary app
Help caregivers to compile required caregiver credentialing documents, answer questions about the program and ensure the caregiver is able to access Seniorlink's app to enable communication between caregivers and coaches
Collaborate with other professionals to communicate circumstances in the home and eligibility for services
Significant travel within the defined service area
What You Will Bring:
Bachelor's degree in social work, or an equivalent combination of education and experience in related fields
Minimum two (2) years of experience as a community health worker or in case management and program operations in the areas of healthcare and social services, including but not limited to working with elders, people with disabilities and other complex medical needs in a healthcare, long-term care, social service or community setting
Proficient and comfortable working with applications on mobile devices
Must have a valid driver's license and a reliable, insured vehicle
