Job Summary:

The Community Outreach and Bereavement Liaison is responsible for providing oversight of Mary Washington Hospice's community bereavement programming and providing education about hospice and bereavement services to the community. The Liaison will spend the majority of their time in the field and must be able to work independently, have flexibility, and manage the workload effectively.

The incumbent in this position will be responsible for compliance with all federal and state laws and regulations governing the privacy and of patients' protected health information.

Essential Functions & Responsibilities:

  • Develops and implements curriculum for community bereavement programs.

  • Conducts community needs assessments for bereavement program design.

  • Coordinates quarterly community information sessions and special events designated for the community bereaved.

  • Responds to all inquiries for the community bereavement support and triages/makes referrals as necessary.

  • Oversees and coordinates peer-led grief support groups for adults and children; including recruitment, training, and supervision over volunteer facilitators. May include evening and weekend hours, as needed.

  • Develops and maintains effective business partnerships with healthcare providers, long term care communities, and community organizations.

  • Initiates and facilitates communication between healthcare providers, community organizations, and Mary Washington Healthcare to ensure awareness of hospice and community grief support programs and serves as an educational resource.

  • Maintains database of community partner contacts and opportunities.

  • Develops surveys and evaluation metrics to measure outcomes/outputs for reporting to funding sources.

  • Performs other duties as assigned.

Qualifications:

  • Master's degree in Social Work (MSW) required.

  • Experience providing counseling related to grief and bereavement required.

  • Must meet the MWHC suitability criteria for operators of vehicles.

  • Reliable mode of transportation required.

  • Supervisory experience preferred.

  • Experience with community resources related to end-of-life issues preferred.
