/A Core Job Responsibilities:Assists with set up and breakdown of site Completes paperwork to register community members as they arrive at testing site Assists with traffic control Performs temperature checks Performs nasal swabs to collect specimens Adheres to policies and procedures for proper specimen labeling, handling, and storage Assures confidentiality of patient information and medical records Collects and organizes supplies to ensure all work areas are restocked appropriately Provides education to community members about the testing and results process Provides consistent and compassionate service to community members Prioritizes and coordinates multiple testing encounters simultaneously Other duties as assigned Physical Demand Level: Ability to function in varying outdoor temperatures while wearing full personal protective equipment (PPE) Ability to stand for extended periods of time #

Additionally, the Community Case Worker is responsible for providing comprehensive direct client care coordination, case management services, coordination of and referral to health and social welfare resources and, client education related to the promotion of healthy lifestyles, disease prevention, and/or chronic disease management. Community Case Worker operates under the direction of the program Case Manager and manages a case load of patients with less intensive needs or on needs as directed by the licensed Case Manager. # The person in this position will function as an integral member of a collaborative and interdisciplinary team, using assessment, coordinating, monitoring, and evaluation skills. They will maintain effective communication with hospitals, physicians, community agencies, etc. and serve as a client advocate, assisting with the navigation and appropriate utilization of health services and community resources.
