Position Objective: Provide oversight and monitoring of the Host Home sites. Ensure the delivery of outcome-based services and supports for individuals served reflecting the application of personal plan. Ensure that home environments operate within established standards including health and safety guidelines, protection of individuals' rights. Will follow Rule 50, Dept. of Human Services Rule 115, 116, 119, 120, and other relevant regulatory guidelines.

Essential Duties/Responsibilities

  • To support the philosophy of the agency's mission, vision, values, and strategic plan.

  • To conduct oneself in a manner that reflects the highest standards of professional courtesy and ethical practices to people served, staff, guardians, families, and all outside contacts.

Program/Department Development, and Coordination

  • Provide OJT and CBTA training for Host Home Providers, DSPs, ensuring maintenance of professional certification.

  • Monitor the delivery of services and supports as outlined in each individual's Personal Plan ensuring the activities and supports reflect individual preferences.

  • Complete mandated Host Home site audits 2x a month, with one being unannounced and ensure host homes are up to DHS' regulations and standards.

  • Assist the Individuals in striving towards independence in accordance with goals and objectives in the Implementation Strategies.

  • Assist in admission, transfers and discharge of individuals.

  • Interact with Community Options, Host Home providers, EP!C staff, people supported, family members, other professionals, and other relevant people in a professional manner.

  • Follow and implement all agency and departmental protocols and procedures.

  • Communicate regularly with the QIDP and other team members regarding progress toward outcomes, health and safety and other issues that may arise at the Host Home setting.

  • Assure dignity, privacy and rights of participants are respected at all times.

  • Complete documentation as required regarding individuals served, quality assurance, and home environment.

  • Participate in Host Home vacations, Power Team activities, fund raising and other events that are included in the Community Options program.

  • Attend Community Options and Host Home meetings, in-services, as directed by supervisor.

  • Perform other duties as assigned.

Marginal Responsibilities

  • Perform other duties as assigned.

  • As a mandated reporter, ensure allegations of abuse, neglect, and exploitation are reported as required by EP!C policy and government regulation

Education/Experience Requirements

  • Minimum of High School diploma or equivalent (GED).

  • Three years experience working with people with developmental disabilities preferred.

  • Valid Driver's License and automobile insurance.

  • Must successfully complete CPR, First Aid, Infection Control, and Crisis Prevention Institute (CPI) training and maintain these certifications.

  • Must complete DSP training in accordance with the state regulations and maintain acceptable status on the Health Care Worker's Registry under the Healthcare Worker's Background Check Act.

  • Must obtain/maintain standing as an Authorized Direct Support Person for medication administration.

Competency Requirements

  • Must be able to assist in the evacuation of residents in emergencies.

  • Must possess emotional stability to handle stressful situations on a regular basis and the ability to cope with illness, death, and dying.

  • Must possess excellent organizational and time management skills.

  • To perform job successfully the individual must be able to perform each essential duty satisfactory and have a flexibility to work evenings and weekends as necessary.

Physical Requirements

  • Frequent walking and sitting.

  • Regular reaching, grasping, and standing.

  • Other physical activities related to an office environment.

  • Operate general office equipment, including multi-line phone, fax machine, printer, copier, and personal computer.

  • Ability to lift 50 lbs.

Work Environment

  • Work environments may vary to include office, home environments, and community settings.

  • Will require travel in regions that EP!C provides support

  • Will be required to drive a motor vehicle (EP!C's, employee's or rental) as part of his/her employment responsibilities with EP!C. A valid Illinois Driver's License and insurance is required and must be maintained at all times while employed by EP!C.
