Luke's is looking for a Performance Improvement Specialist RN- LTC/SNF and INR to join us in our strategy to be the community's trusted partner in providing exceptional, patient- centered care.

The Performance Improvement Specialist RN- LTC/SNF and INR will implement performance improvement and quality programs to leverage subject matter expertise across multiple settings and operating units in support of SLHS mission, vision, and strategic objectives. This role will support the strategic and operational objectives for Long Term Care (LTC)/ Skilled Nursing Facility (SNF) and Inpatient Rehab facilities within St.

Luke's Health System. The role applies competent working knowledge of subject matter; typically handles more complex issues and applies independent decision making; deals with long-term/bigger picture goals and objectives and works independently in leading multi-department teams and facilitates performance improvement initiatives. The Performance Improvement Specialist RN- LTC/SNF and INR will assist staff and leaders with data analysis to identify meaningful performance improvement opportunities, measure performance and establish measurable performance goals.

Required Criteria

  • Education: Bachelor's degree in Nursing (BSN)

  • Experience: 2 years of relevant experience

  • License/Certificate: RN License in state of practice
