Shift: 730am-330pm , 330pm-1130pm, 1130pm-730am

Requirements:

• Active CNA certification from New York
• 6 months of experience within a skilled nursing setting
• High School Diploma or its equivalent

Job Description:

• Endorses, interprets, and enforces the objectives and procedures of Isabella and the Nursing Department
• Plans, directs, coordinates, delegates, and evaluates the activities of all nursing personnel within assigned area
• Conducts daily resident rounds to observe and evaluate the physical and emotional status of each patient
• Reviews resident care plans for appropriate goals, problems, interventions, and revises as needed, based on identified nursing needs
• Ensures the continuity of resident care by informing supervisors and staff of patient status and related matters through written reports and verbal communication
• Conducts daily conferences with unit staff to review specific patient care, and to communicate and interpret recent policy and procedure changes
