PURPOSE OF YOUR JOB DESCRIPTION:

The primary purpose of your job as a member of the nursing staff is to assist the nursing staff under the supervision of a licensed nurse. You will participate in a wide range of patient care activities, for a mainly geriatric population, functions and services for assigned patients, which include direct care and task oriented assignments.

DELEGATION OF AUTHORITY:

Reports to the Charge Nurse or designee.

PERFORMANCE REQUIREMENT / DUTIES AND RESPONSIBILITIES

Implements patient care:

Implements clinical and technical aspects of care and nursing orders in accordance with established policies and procedures. Intervenes with proper technique, procedure and safety precautions to meet individual needs of patient

Implements physician's orders

Documents nursing intervention, patient response, effectiveness, complications, etc. communicates information to licensed nursing personnel

Consults with all available in-house resource personnel and departments for treatment

Respects dignity and confidentiality of patient acting as an advocate

Provides a safe and secure environment for patient

Functions within limits of hospital and nursing department policies and procedures

Performs in a calm and professional manner during crisis and/or emergency situation

Accountability: Quality of work

Adheres to nursing dress code and wears identification pin

Demonstrates organizational skills in providing nursing care by ability to adopt changing workloads

Daily assignments are completed within an 8-hour shift without compromising the quality of care

Performs according to policy:

  • Temperature

  • Pulse

  • Respiration

  • Blood pressure

  • Weight/height

  • Prepares patient rooms on admission, daily, and upon patient request

  • 12 lead EKG

  • O2 sat

  • Pulse oximeter

  • Patient personal hygiene

  • Positioning and transferring of patients

  • Specimen collection

  • Glucometer

  • Patient transportation

Maintains open communication channels with all colleagues and supervisors

Communicates factual and pertinent information in report

Documents according to policy

ADL flow sheet

  • Vital signs

  • weight

  • Blood sugar

  • Intake and output

Restraint checklist

May be assigned additional tasks such as re-stocking rooms, equipment maintenance, patient transportation

EDUCATION REQUIREMENTS:

  • High school graduate or equivalent

  • Massachusetts CNA License

  • BLS

EXPERIENCE:

  • Previous medical/surgical experience preferred

PERFORMANCE REQUIREMENT / DUTIES AND RESPONSIBILITIES: ASSESSES PHYSICAL, PSYCHOLOGICAL, AND SOCIAL DINEMSIONS OF THE PATIENTS: Applies knowledge of illnesses, injuries, and diseases and uses available resources in assessment process. Provides written assessment of patient on admission documents nursing intervention, patient response, effectiveness, and complications and summarizes information on discharge. Summarizes assessment data and makes a nursing care plan which is written and communicated to other patient care providers. IMPLEMENTS PLAN OF CARE: Implements clinical and technical aspects of care and nursing orders in accordance with established policies and procedures. Intervenes with proper technique, procedure, and safety precautions to meet individual needs of patient. Functions within the limits of State Nurse Practice Act, ENA, hospital and nursing department policies and procedures. PATIENT AND FAMILY INSTRUCTION/EDUCATION AND DISCHARGE PLANNING: Assesses, plans, and implements instructions for patient & family needs. PROFESSIONAL LEADERSHIP: Reports patient care or management problems to Clinical Coordinator and/or supervisor when necessary and investigates and intervenes in problem situations reporting appropriate information. Makes out-patient assignment based on acuity and staff qualifications and experience. Coordinates unit routines, providing guidance to staff, orienting new staff, assuring presence and working order of emergency equipment. UNIT REQUIREMENTS: EMERGENCY ROOM Understands and implements the principles of Emergency Room triage. Recognizes the need and initiates the proper sequence of notification when a disaster is anticipated. Demonstrates a knowledge of trauma, cardiac, neurological, orthopedic, thermal injuries and other emergencies including appropriate therapies, special procedures & equipment. Demonstrates special knowledge of: Airway management(glidescope/ett/lma) Lucas cpr compression device Level 1 rapid infuser Pyxis Rigid cervical collar Defibrillator/pacer/synch cardioversion Arterial line set-up Chest tubes Tourniquets Intraosseous Catheters Suture Sets Demonstrates a knowledge of drug specific to the ED PERSONAL GROWTH AND DEVELOPMENT: Identifies own learning needs and seeks opportunities for self-growth, using and sharing skills gained through continuing education classes. Has completed annual nursing competencies. Has attended annual mandatory day. AGE SPECIFIC COMPETENCY: Evaluates for age-appropriate behavior, motor skills, and physiological norms of patients, recognizing a patient's psychosocial development and modifies approaches for patients. Interview and communication techniques are age-appropriate. GENERAL REQUIREMENTS Respects others. Protects and preserves confidential information. Communication skills.
