Position Summary:

Provides quality personal care to clients in home setting. Assists clients with Activities of Daily Living. Performs combination of duties, under the direction of clinical staff to maintain and promote healing. Recognizes disease process, participates in assessing, treating and controlling skin breakdown. Promotes positive relationships with client/family and other agency staff. Adheres to agency policies and procedures. Participates in disaster preparedness plan as needed.


  • Graduation from High School or equivalency is required

  • Must have Florida CNA Certification Medication Assistance training (4 hour course) is strongly preferred. If aide does not have upon hire, must obtain within 1st 90-days of employment.

  • Medically related background is helpful.

  • Current CPR certification

  • Reliable transportation

  • Valid driver license and auto insurance required.

Main Duties & Responsibilities:

  • Provides oral hygiene, bath (tub, shower, bed) shampoo, sitz bath, linen change, light meal preparation, feeding assistance.

  • Provides skin care including back rubs, foot care, etc.

  • Keeps skin clean and dry.

  • Reports reddened areas.

  • Assists clients with ADL'S.

  • Assists client to transfer and/or ambulate.

  • Uses good hand washing and aseptic techniques.

  • Adheres to infection control policies and procedures.

  • Keeps the client's area clean.

  • Assists with medications that are ordinarily self-administered (as allowed according to state regulations).

  • Utilizes healthful preparation and storage techniques in food handling.

  • Provides Foley catheter care as assigned based on agency policies.

  • When appropriate, ensures that bed is in low position and locked at all times and that side rails remain in up position.

  • Takes and records blood pressure, temperature, pulse and respiration. Conducts other simple procedures as assigned and trained by a licensed nurse or therapist.

  • Measures and records client's intake and output.

  • Submits documentation to the agency in a timely manner according to agency policy and procedure.

  • Reports any problems, concerns or unusual occurrences to the agency in a timely manner.

  • Promotes positive professional relationships with the client/family and other agency staff.

  • Refrains from prohibited activities such as changing sterile dressings, irrigating body cavities, catheterizing, administering medications, applying heat by any method, caring for a trach tube or providing any personal health service not included in the Plan of Treatment.

  • Demonstrates safety precautions in compliance with OSHA, CHAP, Federal and State standards

  • Operates medical equipment correctly for client care.

  • Orders supplies for clients in coordination with other treatment team members.

  • Assists in reminding the client to self-administer medications.

  • Maintains an understanding of the company's scope of services. Understands the infrastructure and how and where to transfer calls in the company.

  • Maintains a professional image.

  • Uses appropriate phone etiquette.

  • Understands the infrastructure of Covenant Home Health Care to appropriately transfer calls within the agency.

  • Promotes a customer friendly atmosphere for all visitors and ensures client confidentiality at all times.

  • Participates in accreditation program.

  • Performs other duties as assigned.

  • Participates in in-service program.

Our PCT roles do not require previous health care experience so consider applying today. *What you can expect:* * *Direct Patient Care.* Spend the majority of your day in direct one-on-one patient care to provide safe, comfortable and hygienic dialysis treatment. Our PCTs care for up to 4 patients at a time. * *Technician Duties*. To monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup. Please note you will have exposure to blood and needles. * *Continuous Improvement:* We are consistently looking for ways to improve processes for our patients and teammates as we aim to build the greatest health care community the world has ever seen. * *Building long-term relationships *with your patients who are suffering from end stage renal disease and are receiving treatment on average 3 times/week. You will also be responsible for educating patients on Kidney Dialysis related topics. * *Team.* Cross functional team of clinicians including technicians, nurses, dietitians, social workers and leadership. PCTs work under the supervision of a RN. * *Schedule*. Shifts can begin as early as 4:30 am and can vary based on patient needs. Your expected shift will be shared during the interview process *Some details about this position:* * Training may take place in a facility or a training clinic with a class of new teammates other than your assigned home clinic * Hours of work may vary during the training period; however, you will typically be working between 32-40 hours per week during training * You must be flexible on the final home clinic placement as it may be in a different location; there is also the potential to float to various clinics during and after your training * You must have a flexible schedule and be able to work mornings, evenings, weekends, and holiday *Requirements:* * Desire to enter the health care field to care for other people in need * High school diploma or equivalent * Must be comfortable working around blood and needles * Must be comfortable mixing acid or bicarb * Flexibility and availability to work mornings, evenings, weekends and holidays as needed * Physical and mental ability to work long hours (some shifts are 12+ hours) on your feet so wear comfortable shoes!
