General Purpose:

To support the common goals of the office as it relates to the management of the day-to-day office operations. To provide personal care support to clients in the home and community settings as permitted by federal and state law and regulations. To assist with coaching/mentoring new care professionals in the field. Assist with education/in-service training, orientation, and special event planning. Assist with filling in shifts and covering field team vacations and assisting with employee retention strategies.

Essential Functions:

  • Position includes both personal care duties and supervising field employees.

  • MUST have a flexible schedule to accommodate staffing/client needs on an as-needed basis including mornings, midday, afternoon, evening, overnight, and some weekend availability.

  • Meets with and maintains phone contact with the care coordinator/on-call office personnel daily, including beginning and end of each day.

  • Complete field staff supervisory visits for 30-day, 90-day, unscheduled, and annual visits.

  • Assist with personnel files to ensure they are up-to-date and in compliance and training complete in timely manner.

  • Assist with orientation and training development programs for all care professionals.

  • Conduct caregiver/client introductions in client homes.

  • Fill-in shifts for census changes, vacations, sick time, and complex cases.

  • Develop and oversee retention programs for care professionals including caregiver appreciation activities and caregiver satisfaction surveys.

  • Assist with budget and special event planning for holidays, birthdays, anniversaries, healthcare events, etc.

  • Observes and reports safety hazards and changes in client condition to Supervisor.

  • Assist with care plan development and implementation.

  • Assist with marketing events and recruitment events.

  • Documents observations and activities in an accurate, complete, and timely manner.

  • Organizes self to carry out visits/shifts and to complete assigned tasks.

  • Attends mandatory in-services and provide requested documents to keep employee file current.

  • On-call rotation schedule with administrative team.

  • Completes other assignments as requested and assigned.

  • May have access to personal health information ("PHI") necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.
