Description

  • Job Summary:

Under the supervision of the Center Manager or other designated clinical care entity, CNA(s) is responsible for attending to the daily living and safety needs of participants and aiding the licensed nursing associates in the care of participants. This includes showering, assistance with feeding, ambulation and toileting. Collaborates and assists Recreation Coordinator with activities within the Adult Day Health Center. Assists with food service delivery to include serving preparation and delivery of meals to participants according to dietary recommendations within the facility. Assists and coordinates with Registered Dietitian to ensure proper and safe delivery of nutritional services.

  • Essential functions and Responsibilities:

  • Meets participant's personal hygiene and basic care needs in a timely, safe, and caring manner.

  • Assists participants with activities of daily living, such as showering, feeding, ambulation, toileting, and other activities.

  • Responds to call lights promptly and fulfills participants' requests or obtains appropriate assistance.

  • Obtains vital signs to include temperature, pulse, and respirations (TPR), Blood pressure (BP) and weights.

  • Collects data and records activity and participant care information within the electronic health record. Ensures information is accurate, timely and recorded according to established guidelines.

  • Observes participants for emotional, behavioral, physical changes in condition including but not limited to any changes in mood, skin integrity, bowel and bladder status, food and fluid intake, speech, ambulation, and transfer ability. Reports changes in participant status to the clinic personnel according to policy requirements.

  • Assist with meal service in providing food and drink for participants, assisting with feeding as needed. Refers to and follows dietary orders for participant meals.

  • Assist with oxygen -reminders to wear/use, move oxygen in the dayroom.

  • Follow and assist with emergency procedures of PACE center.

  • Assist with transfer and follows transfer and ambulation order of participants.

  • Managing meal deliveries (bagging meals, coordinating with transportation & receptionist, meal documentation)

  • Managing supplement deliveries (ordering supplements, coordinating transport, documentation.

  • Oversee meal service & making of frozen meals (storage and organization).

  • Assist RD with sensor placement and meter downloads.

  • Assist RD with weight tracking for monthly weight report.

  • Communicate effectively with RD and assist in any needed nutritional related tasks.

  • Follows all procedures and policies for equipment use and monitors supply use in a cost-efficient manner, alerts management of concerns or needs regarding policies, procedures, or practices; follows correct channels of communication.

  • Ensures participants are comfortable and privacy is maintained in all interactions, including other participants and staff.

  • Functions as a member of the interdisciplinary team. Maintains regular attendance at, and participation in, interdisciplinary team meetings; collaborates on care planning and service allocation decisions.

  • Completes all documentation requirements in the medical record in a timely fashion, according to policy.

  • Supports Quality Improvement efforts.

  • Protects privacy and maintains confidentiality of employee, participant, and sensitive agency information.

  • Supports agency's mission and values.

  • Performs other job functions as necessary.

III. Qualifications and Requirements:

Education: High School Diploma or GED. Certification as a Nursing Assistant or Home Health Aide with current credentials will be deemed qualified. Completed training and received ServSafe certification within 90 of hire.

Experience: A minimum of one-year experience working with frail and elderly population is required. One-year experience within a hospital, home care or nursing facility preferred. "Comparable experience" should include skills such as 1) Temperature, Pulse and Respirations (TPR); 2) Blood pressure (BP); 3) bed baths/showers; 4) Activities of Daily Living (ADL); 5) Coordinating delivery of nutritional services to meet participant needs.

Spends majority of time standing, walking, bending and stooping (80%); sitting (20%).
