Community Access Individual (CAI)

A Community Access Coordinator provides assistance out in the community doing activities of the Individuals' choice. Activities outside the Individuals home in a community-based setting 1:1 staff to individual ratio.
CAI will help Individuals to retain or improve self-help, socialization and independent functioning outside the home.
We are dedicated to working diligently with Individuals and families to identify strength, needs, and work towards achieving individual's hopes, dreams, and desire.
Grooming (Assist if needed, to make sure appropriate while in community)
Assistance with toileting (If needed while in community)
Encouraging physical activity ( If required during CAI such as exercising/Gym)
Applying first aide in case of sudden illness or an accident
Providing watchful supervision and oversight
Assist client with shopping
Assist client with activities within the community
Assist client with ISP goals
Participate in the implementation of the CAI Goals
Documentation Requirement: Must document the following in the record of each participant receiving Community Access Services:
Specific activity, training, or assistance provided;
Date and the beginning and ending time when the service was provided;
The location where the service was delivered;
Verification of service delivery, including first and last name and title (if applicable) of the person providing the service and his or her signature;
Progress towards moving the participant towards independence by meeting the participant ISP, which includes person-centered goals, desired outcomes in the participant's action plan, and the amount/type of assistance/support in the Current Service Summary and the Health and Safety sections of the ISP.
To apply you must meet the guidelines listed below:
Must be 18 years or older
Proof of Legal authorization to work in the United States
High School Diploma or Equivalent
Current CPR(Click here to get CPR certification)
Current First Aide Card(Click here to get First Aid certification)
Current Physical (Can be obtained at local Health Department)
Current TB or Chest X-Ray
Valid GA Driver License
