variety of services related to the administrative function of MMHS's Population Health program. The Specialist will work as an integral part of an interdisciplinary team seeking to achieve optimal clinical outcomes, assisting patients with timely and appropriate care access and improving patient satisfaction. The Specialist will accurately and timely prepare reports in final form, collect quality measure and health equity data (inclusive of HEDIS metrics) for reporting, identifying care-gaps and KPI tracking, schedule meetings, take meeting minutes, conduct research, and special assignments as prescribed by the manager.

PERFORMANCE DIMENSIONS AND TASKS

ESSENTIAL FUNCTION

1.Provides ongoing support and resource information to the Population Health Manager.

2.Assist with projects including care team design and patient empanelment, the creation of condition-specific registries, and patient outreach and engagement and soliciting patient feedback, experience and preferences with attention to sub-populations and those with identified disparities including assigned patients not yet seen by the clinic.

3.Assist with validating established EMR reports and conduct relevant data mining and analysis with end goal of improving health outcomes and providing actionable data to Manager, clinicians, team members, and administration or other stake holders.

4.Assist Manager with development of care coordination program and establishment of care team including workflow and personnel planning.

5.Support Manager with gathering materials and data to meet project objectives and timelines.

6.Demonstrate familiarity with Value Based Care and Population Health concepts.

7.Design and maintain dashboards on quality, population health management measures and other metrics.

8.Translate quality, operational and technical data and analysis into understandable and actionable information for diverse audiences.

9.Document and track Population Health and outreach campaigns and associated call and visit volumes.



  1. Assist as part of clinic team by answering patient questions about clinic services and access and scheduling visits.

  2. Assist with Social Determinants of Health data tracking and analysis to identify gaps and disparities among patient population.

  3. Perform various clinical tasks including meeting preparation, recording minutes, photocopying, compose routine correspondence and responding to telephone messages.

  4. Participates in complaint investigations and reporting when necessary.

  5. Communicates effectively to the Manager and solves problems efficiently and effectively.

  6. Prepare confidential and statistical reports.

  7. Builds and maintains effective working relationships with others. Deals with patients, physicians, employees, and visitors in a friendly and cordial manner.

  8. Support staff under Population Health Manager including Patient Care Coordinators, CPSP Workers, Dietician and others through various activities including committee meetings, workgroups and focused projects.

  9. Exhibit a strong ability to work both independently and as part of a team, taking initiative in various projects.

  10. Performs other duties as assigned.


QUALIFICATIONS

Education: High School Degree/GED equivalent, Required

Associate's Degree Preferred

Experience: 2 years of directly related experience, Preferred.

Evidence of high performing clerical work as in spreadsheets, Word documentation, taking Minutes, scheduling, etc.

Advanced data analysis skills, Preferred

Licensing Requirements: Clinical background, Preferred.

Language Requirements: Bilingual English/Spanish, Preferred

Physical Demands: Must be able to sit for 6 hours and stand/walk for 2 hours or more per day. Must be able to bend, reach overhead and kneel. Must be able to push/pull up to 50 pounds. Manual dexterity is necessary for each and skillful use of the hands when working with equipment.

Special Demands: Ability to work well with others and to perform a variety of assigned duties.
