The primary functions of the Clinical Team Leader include oversight of all patient care and ensuring all home health services are coordinated in a safe and timely manner. The role is responsible for review and approval of all patient information during the start of care, re-certification, resumption of care and/or evaluation visits. The role is also responsible for review and approval of orders, assistance with case conference, and assisting the Branch Director with day to day clinical and agency operations management including routine education and training for branch clinicians as needed.
Day to day clinical operations management including:
Process workflow tasks, coordination notes, and administrative tasks
Review infection control reports and occurrence reports, and document all follow up appropriately
Initiate employee and patient infection reports, as necessary
Review of on call coordination notes reports
Management of referrals including:
Referral entry completion
Scanning referral paperwork timely
Non-admitting prospective patients, as needed
Serving as back up to Intake Coordinator (if applicable)
Assist and support the Branch Director including:
Run and print reports as needed during survey
Move appropriate patients in system to survey team for review and move back to original team following survey
Resolve provider-specific requirements (i.e.: NPI, PECOS)
Support and back up Patient Services Coordinator including:
Reschedule missed / declined visits
Process reassigned and reschedule requests
Follow up on unsigned orders as needed
Manage referrals including:
Ensure patients receive timely assessment / evaluation visits
Enter detailed non-admit data in coordination note if applicable
Ensure non-admits are approved by Branch Director
Manage and oversee patient care including:
Review and approval of all patient information submitted during start of care, recert, resumption of care or evaluation visits
Management of physician orders
Review and approve orders, as needed
Route orders to Medical Records to be sent for physician signature
Update the patient's medication profile and schedule as needed via the orders
Ensure orders that address frequency have appropriate calendar modifications
Ensure orders are in place for requested medical supplies
Assist Branch Director with case conference including:
Complete recert/discharge decisions and document via coordination notes timely
Review and approve case conference coordination notes submitted by licensed professionals
Contact physicians to obtain orders as needed
Review and process clinical information including:
Process vital sign alert reports
Document action taken for vital sign alerts including follow up and physician notification
Review and process wound score deviations
Receive lab reports and assess for normality; fax lab report to physician with signature indicating review; scan reviewed labs and fax confirmation page to Medical Records for uploading into patient chart
Perform review of evaluation documentation, OASIS and Plan of Care including:
Ensure consistency with comprehensive assessment and the Plan of Care
Follow up on documentation that requires correction
Ensure service utilization correlates with assessment data
Process unlisted supplies, medications, activity, functional limitations, and allergies
Verifies correct start of care and episode date range
Ensures appropriate Care Types and NDPs are selected
Assist Branch Director with conducting OASIS and documentation orientation
Perform patient visits and/ or participate in on-call rotation as needed
Must be a graduate of an approved school of professional nursing and licensed in the state of residence.
Supervisory experience is preferred.
Must possess a valid state driver's license and automobile liability insurance.
