The Clinical Team Leader (CTL) is responsible for the daily clinical coordination and support of staff providing hospice care to the patient/family unit as indicated in the plan of care. Provides and manages direct care to patients.

Qualifications:

  • Current Florida or multi-state license to practice as an RN, and ability to apply for/obtain multi-state RN license within 30 days of hire, if not already a holder of a multi-state license

  • Ability to apply for/obtain license to practice as an RN in Washington D.C. within 30 days of hire.

  • Active BLS for healthcare professionals from the American Heart Association or Red Cross.

  • Minimum of two (2) years' nursing experience

  • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system

  • Mobile Driver - Valid driver's license and automobile insurance per Company policy

  • Excellent critical decision making and problem-solving skills

  • Ability to use equipment with visual and auditory mechanisms

  • Ability to effectively communicate in English

  • Flexible schedule to meet business needs and extended hours oversight

  • Ability to manage the emotional stress of working with and caring for terminally ill patients and their families

  • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation

Job Responsibilities:

  • Provides and manages patient care to patients and families in the Hospice Help 24 department incorporating psychosocial, spiritual, cultural, physical and biological components, and appropriate nursing intervention and follow-up.

  • Coordinates employees and patient care to ensure that the needs of the patient and family are met using best standards of practice and in an efficient manner.

  • Recommends alternate approaches to patient care as indicated while ensuring that policies and procedures are followed.

  • Provides education to team members regarding hospice/palliative philosophy, symptom management, case management, and organization

  • Enhances the clinical practice of team members by performing as a role model, mentor, instructor, and resource person. Primary clinical resource for department staff.

  • Collaborates with other departments (i.e., Medical Services, Pharmacy, Infusion Program, and Continuous Care) to ensure patient care needs are addressed.

  • Demonstrates respect and understanding for all clinical disciplines required for quality patient care and mentor staff in this process.

  • Communicates in a clear and accurate manner Provides constructive feedback and resolves conflict directly. Assists others to improve communication skills. Communicates accurately and completely to physicians, staff members, patients, families, and supervisors; utilizes positive approaches when working with others.

  • Ensures timely attention to customer concerns and complaints.

  • Provides excellent customer service to both external, and internal customers.

  • Assists in reviewing patient care documentation and providing clinical supervisory visits and clinical competencies to identify strengths and areas of development for the clinical staff.

  • Triage RN's who experience technical issues not caused by the company equipment or network (i.e. offsite power outage, personal internet outage) are required to report the situation to leadership immediately, and have 30 minutes to resolve the technical issue. If the issue cannot be resolved in 30 minutes, RN may be required to report to a IPU or Resource Center based on leadership direction.

  • During times of emergencies (i.e. Hurricanes, etc.), the Triage RN may be required to report to work at a location designated by the company, to ensure continuity of services. This may include reporting to work ahead of your scheduled date/time due to planned lock down of unit, and staying overnight(s) based on duration of emergency.

  • Performs other duties as assigned.
