Our client is ahome health agency,the Clinical Supervisoris responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Supervisor is responsible for the clinical activities of the field staff. The clinical supervisor plans, implements and evaluates patient's plan of care for appropriateness to the individual patient's needs. Great promote from within opportunity too!
  • Assesses and supervises the delivery of nursing services
  • Performs home supervisory visits of nursing services delivered by licensed nurses, home health aids, and other nursing support staff
  • Reports changes in the patient's medical or mental condition to the physician and the Director
  • Reviews patient's clinical diagnosis, medications, procedures and clinical course
  • Assists clinicians in establishing immediate and long-term therapeutic goals
Who Will Be a Great Fit?
  • 3+ years of RN experience in a home health agency, primary care clinic or health facility.
  • Bachelor's degree in nursing
  • Proven ability to supervise and direct professional and administrative personnel.
  • Current State License, CPR Certification
  • Will consider extremely strong home health RN looking for an entry level leadership role

VIEW JOBS 11/18/2018 12:00:00 AM 2019-02-16T00:00 The primary purpose of this position is to assist the Administrator in the planning, organizing, and directing of patient care services in accordance with current standards and regulations so that the highest degree of quality care will be maintained at all times. The Clinical Manager directs patient assignments, staff scheduling, coordination of referrals , patient visit volume distributions, and assists in meeting financial targets by effectively managing utilization, productivity, personnel and supplies. The Clinical Manager ensures adherence to clinical best practices to promote optimal patient outcomes and patient experience and ensures that patient needs are continually assessed through development, implementation, and updates to the individualized plan of care in coordination with the Case Manager. The Clinical Manager participates, as necessary, in team meetings and represents clinical staff on appropriate committees and the agency to the community. The Clinical Manager is responsible for the oversight of a clinical multi-disciplinary team of Registered Nurses, Licensed Vocational Nurses, Physical Therapists, Physical Therapy Assistants, Occupational therapists, Speech Language Pathologists, Occupational Therapists, Medical Social Workers and Home Health Aides, and manages an active patient census of approximately 200 patients. The Clinical Manager supervises, supports, and educates staff at the point of service in the field. The Clinical Manager supports an efficient and accurate revenue cycle by enhancing capture of appropriate revenue and managing the flow of documentation within their department. The Clinical Manager may provide direct patient care in times of high census and/or for emergent need and takes on other managerial tasks and projects as assigned by Administrator. Education AA in Nursing require, BSN Preferred or bachelor's in physical therapy required. Licensure Registered Nurse - CA RN or Physical Therapist - CA PT Basic Life Support Certification - BLS Experience Three to five years of experience in home care agency or two years of supervisory experience in an acute or similar setting required. Minimum two years experience in own professional discipline within the last five years Skills and Knowledge Must have excellent interpersonal skills and role model customer service skills Must establish and maintain positive and effective working relationships with staff, physicians, patients, and other departments Must establish and maintain positive and effective working relationships with staff, physicians, patients, and other departments Self-starter with a high degree of initiative, motivation, flexibility, and creativity At orientation and annually thereafter, must be able to meet core competencies for position and maintain licensure and professional development requirements as appropriate.
