Job Description and Duties
Under the supervision of the hospital Chief of Social Work Service, and in consultation with the statewide DSH Program Director for Discharge and Community Integration (DCI), the Discharge and Community Integration Specialist is a coordinator of hospital-based discharge efforts for civil and forensic patients within DSH, including but not limited to: training, resource development, strategic partnership, program implementation & evaluation, ongoing data collection and monitoring, and policies and procedures. Collaborates with the Forensic Conditional Release Program (CONREP) as it relates to discharge and community integration of relevant legal commitments and implementation of the new referral process. Serves as a trainer for staff, and as hospital liaison to the statewide Discharge and Community Integration program. Occasional, limited travel may occur and reimbursement will be in accordance with BU agreements and standard travel policies.

Working Conditions
Occasional, limited travel may occur and reimbursement will be in accordance with BU agreements and standard travel policies.
Hybrid telework is available based on operational needs and is subject to change, with a current minimum three in-office work days per week requirement. In office work requirements for new employees may increase during the first thirty (30) days of employment based on training needs.
The work hours for this position will be set sometime between 8:00 am and 5:00 pm Monday through Friday.

Knowledge and understanding of the Conditional Release Program (CONREP) as well as other mental health community treatment programs
Knowledgeable of the systems of care related to older adults, mental health, substance abuse, developmental delay, primary care, palliative care, chronic illnesses, dialysis, conservatorship, and probation and parole
Knowledgeable of housing resources, their requirements for eligibility, and other community services such as In-Home Supportive Services(IHSS), Multipurpose Senior Services Program (MSSP), Institute on Aging (IOA), and shelter Navigator and Project Home Key
Knowledgeable about local, state and federal benefit and entitlement programs, such as Social Security, SNAP, Medi-Cal, and General Assistance

Proof of Vaccination Status Information
All Hospital Locations:
Effective October 1, 2021, in accordance with California Department of Public Health (CDPH) State Public Health Order of August 5, 2021, all workforce members in DSH state hospitals are required to be fully vaccinated with a COVID-19 vaccination regimen. Prior to your first day, you will be required to fill out a COVID-19 Vaccination Consent Form.
Effective March 1, 2022, in accordance with CDPH State Public Health Order of December 22, 2021, hospital workforce members currently eligible for booster doses must have received their booster dose.
If you are fully vaccinated, you must provide evidence of vaccination prior to your start date.
If you are not fully vaccinated, you have the option to receive a vaccine from one of the State Hospitals prior to your start date.
If you are not fully vaccinated and choose to complete a request for an exemption, you must submit this exemption and receive approval prior to your start date.
If you are not yet eligible for a booster, you must be in compliance no later than 15 days after the recommended timeframe for receiving the booster dose.
