The Dementia Care Collaborative within the Division of Palliative Care and Geriatric Medicine provides education, support and resources to patients and caregivers facing a diagnosis of Alzheimer's or a related disorder. Our services improve the quality of dementia care in primary and specialty care settings by addressing the needs of the patient and caregiver dyad. With this additional support as part of a comprehensive interdisciplinary team, the patient and caregiver are informed and supported throughout the continuum of illness.
Our focus is to help patients and their families live as well and as fully as possible when facing a diagnosis of Alzheimer's and related disorders.
Primary responsibilities include psychosocial assessments, serious illness and caregiver counseling, advance care planning, symptom management and disease education and acting as a collaborative member of the interdisciplinary team. Knowledge of appropriate community resources and supportive services for patients and their families a plus. Must have strong clinical, interpersonal and organizational skills.
MSW degree from an accredited program
Current Massachusetts license at the LCSW level; LICSW preferred
Minimum of 3 years of health care social work and dementia care related experience required.
