Multnomah County Health Department's Behavioral Health Division is seeking passionate social workers and mental health professionals who represent the communities we serve to join our team as a Clinical Services Specialist

  • Wraparound Care Coordinator. Are you dedicated to removing barriers to youth and families who experience intersectionality? As a Wraparound Care Coordinator, you will bring together children, family members, natural supports, and service providers to lead a multi-disciplinary team in creating individualized plans that address each youth's unique needs. You'll coordinate monthly team meetings to engage in an intensive, team-based planning process. This is an opportunity to support youth and families facing complex challenges related to emotional, behavioral, developmental, medical, or substance use needs by building on family strengths, collaborating with community resources, and incorporating shared decision-making.

We have two (2) Clinical Services Specialists positions to fill out of this recruitment; one has a preference for Substance Use Disorder experience and the other has preference for transitional aged youth/young adults.

Clinical Services Specialist - $1,500 Hiring Incentive

As a newly hired Clinical Services Specialist, you will receive a one-time New Hire Recruitment and Retention Incentive payment of $1,500 on your first paycheck. The incentive will be prorated based on the position's FTE status

Teleworking/Hybrid: At this time, this position is designated to "routine telework/hybrid," meaning you will be working remotely (from home) most of the time and you will be required to attend meetings in the community. As with all telework positions, employees may be required to come into the office on occasion. The designation of Routine Telework may be subject to change at a future time. Currently, all employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations.

Note: The eligible list created from this recruitment may be used to fill regular, full or part time, temporary, and limited duration assignments.

As the Clinical Services Specialist

  • Wraparound Care Coordinator duties include:

Wraparound Care Coordination

  • Facilitate the Wraparound process, including client assessment, clinical evaluation, and treatment planning.

  • Collaborate with youth, families, and service providers to create individualized care plans.

  • Lead monthly team meetings to assess client progress, review care plans, and adjust strategies as needed.

  • Provide exceptional care coordination for complex cases, procuring necessary treatment and coordinating service delivery.

  • Review authorization requests, conduct utilization management activities, and collaborate with system partners.

  • Attend regular coaching sessions and PSU System of Care Institute training.

  • Respond to new referrals within one business day and facilitate timely investigatory meetings.

  • Maintain communication with the Wraparound team, ensuring timely distribution of care plans and scheduling consultations as needed.

Documentation

  • Complete CANS assessments within 30 days of enrollment to determine client needs and inform care planning.

  • Update CANS scoring sheets and Plans of Care regularly based on ongoing evaluations and team collaboration.

  • Maintain detailed service notes in Evolv, adhering to program and state requirements.

  • Document investigation and evaluation findings accurately and submit documentation promptly.

Health Share of Oregon, Medical Necessity Reviews, and Process

  • Adhere to Health Share of Oregon Utilization Review Guidelines when reviewing treatment authorization requests.

  • Review clinical documentation, provide consultations, and offer recommendations for services and supports.

  • Stay updated on administrative processes and track authorization expiration dates.

  • Lead discussions on the current level of care recommendations during Wraparound team meetings.

  • Process authorization requests within 14 days, providing clear explanations and resolving conflicts as needed.

  • Offer consultation to community members seeking assistance.

Workplace Requirements and Responsibilities

  • Complete required County and State training, and adhere to workplace policies and procedures.

  • Participate in supervision, training, and meetings to support professional development and program goals.

  • Maintain professionalism, ethical conduct, and respect for diversity.

  • Submit accurate and timely documentation, including service notes and care plan updates.

  • Attend program-specific meetings, consultations, and training.

  • Follow policies for reporting absences and schedule adjustments.

  • Participate in County-wide training to enhance skills and contribute to the community.

Required Minimum Qualifications/ Transferable Skills*:

Education/Experience:

  • Equivalent to an Associates degree in social services, psychology, sociology, counseling or related field and four (4) years professional work experience as a counselor, advocate, or social worker ; OR,

  • Equivalent to a Bachelor's degree in counseling, social services, psychology, sociology, social work or a related field and two (2) years of professional work experience as a counselor, Mental Health treatment professional, social worker, or therapist; AND

Certification:

  • Must meet the standards of Qualified Mental Health Associate - (QMHA-R) with the Mental Health & Addiction Certification Board of Oregon (MHACBO) - more information can be found at MHACBO website. Current QMHA-R (Required)

Other:

  • Valid Driver's License

  • This position requires a Background Investigation, which may include being fingerprinted.

Preferred Qualifications/ Transferable Skills*: You do not need to have the following preferred qualifications/ transferable skills to qualify. However, keep in mind we may consider some or all of the following when identifying the most qualified candidates. Please clearly explain on your application how you meet any of the following preferred qualifications/transferable skills.

  • Working knowledge of Google Suite, WrapStat, EvolvNX, PhTech/CIM3, MMIS, and/or GSI databases preferred.

  • Working experience as a Wraparound practitioner i.e., care coordinator, supervisor, peer partner, or coach.

  • Experience working with transitional aged youth/young adults.

  • Knowledge of children's system of care.

  • Experience working directly with youth and families experiencing substance use disorders.
