Clinical Services Coordinator - Dialysis

• Bachelor's Degree in Nursing from an accredited college or university

• Master's Degree In Nursing

• Adult Nurse Practitioner

• Five (5) years of post Master's Degree experience As Adult Nurse Practitioner, including experience in providing primary care to children of a variety ages.

• Current New York State Licensed Registered Professional Nurse

• Current New York State Licensed as Nurse Practitioner

• Adult Health.

• Current Basic Cardiac Life Support (BCLS)

• Ability to perform assigned point of care testing (ex. Glucometer, Rapid Covid Testing).

• Ability to administer PPD as ordered and document readings.

• Computer application skills

• ALLSCRIPTS, EPIC, Microsoft Excel.

• Excellent written and communication skills.

• Nursing and leadership skills. . T

• Knowledgeable in systems improvement and have the ability to facilitate a culture of a "Patient Centered Medical Home".

• Empathic with attention to detail, exceptional teamwork skills

Under the supervision of the Medical Director, the Clinical Services Coordinator

• Dialysis is responsible for quality and completeness of assigned medical services for primary care for the dialysis patient population. Assesses, treats, and appropriately referspatients to specialty care or diagnostic services. Provides primary health care services consistent with the philosophy of The Brooklyn Hospital Center.
