As our Clinical Risk Manager, you will review, analyze, and trend segment clinical high risk metrics and reports from all company systems. You will prioritize and trend top risks, report outliers, with recommended corrective action, setting deadlines for segment and monitor improvement. Additionally, your role will include conducting facility site visits, testing clinical high risk systems and segment metric reports, including QA and especially on high risk facilities. Your role will also include conducting monthly High Risk Education for RVP and Signature Care Consultants (as well as buildings, regions, etc. as needed). Thorough review of clinical processes, as warranted on facility site visits, to ensure clinical systems and processes are in place to identify and provide mitigation recommendations of potential high risk areas, best practices, and consultation and training opportunities are additional responsibilities of this role. You will also participate in high risk investigations, as warranted and assigned by segment COO or Home Office Risk Team and collaborate with facilities, regions, segments and home office regarding quality assurance performance improvement analytics and action plans.
Requirements for Consideration include:
RN or other equivalent professional healthcare care provider degree, with license in good standing.
Minimum of three (3) years' experience in long term care and healthcare risk management preferred.
Possess working, firsthand knowledge of State and Federal guidelines.
Extensive knowledge of the Minimal Data Set (MDS), Care Planning, and quality assurance / QAPI processes.
Proven ability to identify, analyze, and trend clinical risk metrics.
Ability to work objectively, independently, and be data-driven
Ability to conduct a true Root Cause Analysis
