  • State licensure required if licensure is available in the state where the facility is located (must have for at least one year)

  • Bachelor's degree in Nutrition, Dietetics, or similar area required

  • Minimum of 12 months' professional work experience in clinical nutrition as a Registered Dietitian required

  • Ability to work flexible schedules, possibly for multiple facilities and travel when needed

  • After hire and training, successful completion of the Competency Assessment for Renal Dietitians (CARD) with a score of at least 80% required

  • Intermediate computer skills and proficiency with MS Word, Excel, and Outlook required as well as functional proficiency with DaVita specific applications within 60 days

Job Duties and Responsibilities:

  • Independently evaluates, develops and completes comprehensive nutrition assessment and reassessment consistent with CMS regulations.

  • Works with patient and IDT to establish, document and update plan of care (POC) to achieve short and long term goals

  • Assesses on-going nutritional status to determine appropriate nutrition intervention(s) and recommendations. Consults physician regarding lab tests as needed.

  • Develops individualized Medical Nutrition Therapy including meal plans for patients based on reported lab results and individualized goals

  • Identifies and develops the treatment and nutritional goals for POC and updates as outlined by the clinic's governing body for stable and unstable patients.

  • May request assistance from Dietitian II/III as needed with advanced or complex clinical renal nutrition situations.

  • Provides nutrition education and counseling based on individualized patient needs including nutrient requirements, knowledge and abilities, medical, economic, and social situation

  • Selects or develops education materials and methods appropriate for individual patient

  • Monitors nutritional status, laboratory values, dialysis adequacy, adherence to diet and response to nutrition related medications and therapy to determine appropriate nutrition intervention and recommendations

  • Documents nutrition progress at least monthly

  • Functions as Mineral and Bone Disorder Manager

  • Reviews monthly lab results with each patient and/or caregiver and provides patient with a printed patient nutrition report

  • Evaluates outcomes and identifies factors that may contribute to undesirable results and documents appropriately

  • Contacts and collaborates with extended care facility staff, or other appropriate medical personnel or caregivers to facilitate adequate and appropriate nutritional care

  • Effectively communicates pertinent nutrition issues to the IDT and to patients

  • Participates in patient care conferences, continuous quality improvement (CQI) teams, and IDT meetings

  • Presents nutrition related in-service for teammates at least annually

  • Travel: up to 30%

Minimum Qualifications:

  • You MUST be registered by the Commission of Dietetic Registration for AT LEAST one year from start date. For example: If your start date is June 1, 2019, you would have become a Registered Dietitian under the commission of Dietetic Registration on or before June 1, 2018.

  • Bachelor's degree in Nutrition, Dietetics, or similar area required

  • Registered Dietitian with at least 12 months clinical experience as required by CMS but with less than 1 year specialized renal experience

  • State licensure or certification required if licensure or certification is available in the state where the facility is located. In states where licensure/certification is optional, a new hire or transfer dietitian will be allowed 120 days grace period to obtain the license/certification

  • Intermediate computer skills and proficiency with MS Word, Excel, Outlook required as well as functional proficiency with DaVita specific applications within 60 days of employment

In collaboration with the patient and family, provides theory-based professional nursing care and coordinates care delivery with the physician and other members of the healthcare team. Qualifications * EDUCATION: Graduation from and accredited educational program in professional nursing. * EXPERIENCE: 3 years of Critical Care nursing experience. * LICENSE/CERT/REG: Valid RN license in the state of Maryland and must obtatin a District of Columbia license before coming off of the 8 week orientation period. Must have current ACLS, PALS, CPR upon hire. Must obtain CCRN, CFRN, or CTRN within 180 days of hire. * SKILLS: Demonstrated ability to precept. Demonstrated high level of clinical competence. Effective interpersonal skills, including verbal and written communication. Basic math skills. Basic computer skills preferred. Primary Duties and Responsibilities * Collaborates with on line medical control physicians to manage the patient care of complex patients in transport. * Communicates patient's clinical information clearly and accurately in verbal and written format. Clearly and accurately documents pertinent information in the patient's medical record. Consistently demonstrates effective documentation practices and develops strategies to ensure complete documentation of patient care. * Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations. * Delivers expert nursing care in ambulances and the Emergency Department to multiple patient types to include adult subspecialties, pediatrics, and high risk obstetrics. Identifies current and potential complex problems of a specific patient population, critically analyzes trends and changes in patient status, and plans appropriate interventions directly or indirectly through other nurses. Assesses needs of patients with highly complex problems. Documents all patient care activities in the electronic medical record. Documents patient information in a manner that reflects the ability to integrate the nursing assessment data, pathophysiology of disease processes, and the therapeutic regimen into an appropriate evaluation of the patient status. * Demonstrates the initiative to seek out current nursing research findings that address patient care issues, and communicates relevant research findings to staff. * Maintains a safe and therapeutic environment of caring for patients and families. Provides basic patient comfort measures during procedures. When necessary, provides environmental support to expedite timely patient admission or transfer. Identifies and acts upon opportunities to improve the environment of care. * Participates in MedSTAR Transport safety program. * Participates in multidisciplinary quality and service improvement teams as appropriate. Participates in meetings, serves on committees and represents the department and hospital/facility in community outreach efforts as appropriate. * Participates in the orientation program for new staff. * Provides direct patient care and evaluates outcomes, based on assessment of physical, emotional, biophysical, and spiritual needs.
