  • File all documentation turned into Medical Records Daily. (See below regarding MARs)

  • Maintain and organize all overflow files. (ICF/IID files must be organized by year due to their size)

  • Examines all pages to ensure that all patient names and patient numbers are correct, check all Medication Records (MARs) for holes, and missing signatures prior to filing in over flow.

  • Organize filing for easy retrieval of documents at the beginning of each day.

  • Breakdown charts, gather all missing items and signatures prior to closing record according to policy and procedure.(IID and Psychiatric Day Treatment, Meadville, Pearl and Newton)

  • Back up Clinical Record Clerk II in the breakdown of charts; gather all missing items and signatures prior to closing record according to policy and procedure. (ICF/IID and PRTF)

  • Conduct performance improvement review, qualitative and quantitative analysis of records on all Day Treatment, and Therapeutic Group Home after discharge.

  • Document specific problems/findings while conducting medical chart analysis, report findings to the Performance Improvement and Medical Record Director.

  • Set-up charts for new admissions for Psychiatric and IID Day Treatment and Raleigh Therapeutic Group Home.

  • Assist offsite facilities (Meadville, Pearl, Newton, and Brookhaven TGH with any request for student/resident information.

  • Complete monthly thinning for all programs according to policy and procedure. ( IID Day Tx week 1, and Psychiatric Day Tx Week 3) Replace any broken charts.

  • Provide care and/or conduct patient/resident interactions in a manner appropriate for the age, culture, and population being served.

  • Follows all safety policies and adheres to all workers' compensation program guidelines.

Responsibilities: Qualifications: * A minimum of a Master's Degree in Speech Language Pathology. * Certification in accordance with the requirements of the American Speech Language Hearing Association. * Licensed by the State in the designated geographic area in his/her respective therapy profession. * Demonstrates the ability to perform the following tasks without posing a threat to the health or safety of him/herself or others without risk to self, co-workers, caregivers, or patient; lift, turn, move position, and transport the patient into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. * Demonstrates the ability to perform a minimum of minimal assist transfer." Minimal assist transfers are defined for purposes of this document as the ability to lift and control 50 pounds. Qualifications: Duties and Responsibilities: * Responsible for the maintenance and accuracy of records in regard to patient care for all patients on personal case load. * Responsible for the correct reporting of billing information of Speech Therapy in regard to all patients on personal case load. * Provide direct patient care as prescribed, including evaluation and treatment planning. * Responsible for the reporting and interpretation of treatment results. * Participate in development and presentation of in-services for the facility as requested. * Supervise support personnel in immediate work areas as appropriate. * Perform clinical functions: screens, evaluations, development of treatment plans, and delivery of treatment in accordance with facility, Medicare, and TMC's policies and procedures. * Participate in meetings regarding patient care as appropriate. * Assist when appropriate with any community education and public relations activities of the facility. * Provide education to patient's family as well as staff on the specific treatment plan as well as therapy goals for the patient. * Responsible for the coordination and cooperation with other staff members in carrying out the treatment plan as established. * Other duties as assigned.
