The Clinical Qaulity Documentation Specialist directs, analyzes, researches and comprehends critical data points to improve clinical competencies and quality assurance of the Company. Provides guidance, support, communication and coordination of clinical programs and application including Care Tracker, MatrixCare and KeySTATS. Reviews all quality data to ensure accuracy and assists with quality improvement plans. Generates reports that assist in the review of data, ensuring that leaders can make clinical decisions on a timely basis. Creates and provides the maintenance of ongoing reports to senior leadership, divisional and campus leadership and the Board of Directors.

Duties and Responsibilities:

  • Conceptualizes the development of all clinical excellence enterprises. Creates strategies and reporting and analytical needs.

  • Provides training and a climate for learning and facilitating the adult learning process of the company's clinical systems (MatrixCare, Care Tracker, etc.) by creating, supporting, planning and presenting educational activities.

  • Participates and identifies strategies for improving professional development activities, clinical system practices, and effective system use by users for quality improvement.

  • Provides clinical content expertise and knowledge of care provider systems to create solutions that will impact quality across the care continuum.

  • Creates and assists in the development of accurate documentation in MatrixCare.

  • Provides quality support. Troubleshoots and assists in creating an action plan for quality improvement.

  • Establishes the strategic direction to incorporate division based multi-disciplinary teams that are focused on reducing variations and improving clinical performance.

  • Assists in Mock Survey for readiness of the state surveys.

  • Utilizes trends, quality data and analysis to identify and design a plan to implement performance improvement initiatives in quality and safety indicators.

  • Provides education related to quality measures, processes and procedures.

  • Designs and conducts analysis that quantifies the impact of policy and/or market changes.

  • Directs automated reporting to provide timely information, allowing decisions to be made in a real-time environment.

  • Reviews medical record documentation for newly admitted residents to ensure all documentation is in place to support the primary medical diagnosis.

  • Uses critical thinking skills to know when to request and communicate on missing documentation essential to supporting the resident's SNF stay.

  • Reviews all infection, fall and wound events and tracks, analyzes trends, and advises management on areas of improvement.

  • Closes all events that are open after a resident is discharged.

  • Develops tools to optimize the efficiency of clinical quality and reporting.

WHAT WE'RE LOOKING FOR

  • College graduate or some college classes preferred.

Experience:

  • Three (3) to Five (5) years' experience in healthcare, senior living industry or long- term care environment

preferred.

  • Navision, Jet Reports and Matrixcare software products or equivalent preferred.

  • Exemplary computer skills that include knowledge of the Microsoft Office Suite of products.

We are looking for a Legal Documentation Specialist to join the Trilogy family! Maintains medical records in accordance with federal and state guidelines as well as established policies and procedures to assure that a complete medical records program is maintained. Ensures quality, accuracy, accessibility and security in both paper and electronic systems. Directs, analyzes, researches and comprehends critical data points to determine missing components of the comprehensive medical record. Provides guidance, support, communication and coordination of documents with multiple legal, operations, and clinical programs. Reviews all quality data to ensure accuracy and assist with quality improvement plans. Duties and Responsibilities * Responsible for gathering, printing, and collecting all responsive pieces of a requested medical record for production to resident, authorized representative, or attorney. This includes direct contact with campuses, including potential travel to campuses as needed, to ensure all records are obtained. * Work with Clinical Risk Manager to assist with identifying potential compensable events. * Work with Campuses to ensure incident workflow and soft files are completed as necessary when notified of certain events. * Utilizes various classification systems to code and categorize patient information for risk and legal reviews. * Assists in the development of procedures to ensure records are properly stored and copied for production requests. * Work with Compliance to ensure proper authority for the production of requested records. * Protects the security of medical records to ensure confidentiality is maintained. * Reviews records for completeness, accuracy and compliance with regulations. * Assist with developing and providing training on record documentation and retention. * Travel could be as much as 50%, including overnight stay as necessary. WHAT WE'RE LOOKING FOR Qualifications: * Bachelor's Degree Experience: * Two (2) to Five (5) years' experience in healthcare, senior living industry or long- term care environment preferred. * Sufficient skills with Navision, Jet Reports and Matrixcare software products or equivalent preferred. * Exemplary computer skills that include knowledge of the Microsoft Office Suite of products.
