To be fully engaged in providing Quality/No Harm, Customer Experience, and Stewardship by being actively engaged in executing, measuring and monitoring the implementation and support of critical clinical processes and services, identifying changes that enhance performance,. Ensure improvements are sustained, and maintain a continuous level of patient safety and accreditation readiness with regulatory agencies. Partner with all members of the Clinical Quality team, leaders, provider offices and associates of all disciplines throughout HFMG sites and IDN to ensure uncompromised safety and practices support superior quality, as well as, optimize delivery of patient care.


Quality/No Harm:

Proactively seek opportunities to assist with the identification of risks to patient safety.

  • Perform LPN Post-discharge condition specific telephone calls, including:

o Validation of Medications across multiple EMRs

o Validation of education on medications and diseases

o Clinical escalation to RN with any clinical changes

o Proactive referrals to support clinical care

o Coordination of care post discharge

  • Support HFMG mandatory quality initiatives.

Support optimization programs from Clinical Quality in partnership with Operational Vice Presidents. Key resource for designated sites on initiatives to improve efficiencies while supporting compliance with regulatory requirements and improved outcomes for both Ambulatory and IDN measures.

Coordinate the indicator monitoring and performance measurement designated sites.

Monitor performance through data collection.

Analyze current performance.

Perform reviews and facilitates the proper use of information.

Provide accurate and timely clinical or medical record review and distributes data to appropriate committees or groups and identifies opportunities for improvement.

Act a resource for data abstraction and analysis as requested by teams.

Coordinate and provide support to clinical teams by providing accurate and timely medical records requests, timely chart review and focus studies as needed.

Provide regional support at designated sites during business hours for Rx, Lab and Diagnostic ordering under Treatment Guidelines or Physician Direction.

Facilitate Quality Improvement and Operational Initiatives as needed under direction of Clinical

Quality Director in partnership with Operational Vice Presidents.

Knowledgeable of team models at designated sites; Act as team facilitator or leader as requested by Clinical Quality Director.

Provide education to team members and staff.

Participates in risk reduction strategies.

Customer Experiece:

Respect and assists all associates and customers appropriately, maintaining patient confidentiality as per professional ethics and HIPAA mandates.


Work within budgetary constraints.


Education: Graduate of an accredited school for LPN Nursing

Experience: 2 years clinical experience preferred

Current Licensed Practical Nursing (LPN) in the State of Florida

Proficiency with Microsoft Office, PowerPoint and other software and/or programs related to role.
