Encourage multidisciplinary collaboration to ensure the appropriate therapeutic use of clinical drugs resulting in optimal patient outcomes.
Proper clinical documentation to maintain patient care and accreditation standards.
Monitors drug interactions and problems, ensuring safety of patients.
Prepares medications by reviewing and interpreting physician orders; detecting therapeutic incompatibilities.
Dispenses medications by compounding, packaging, and labeling pharmaceuticals.
Provides pharmacological information by answering questions and requests of health care professionals counseling patients on drug therapies.
Develops staff's pharmacological knowledge by participating in clinical programs; training pharmacy staff, students, interns, externs, and health care professionals.
Complies with state and federal drug laws as regulated by the state board of pharmacy, the drug enforcement administration, and the food and drug administration by maintaining records for controlled substances; removing outdated and damaged drugs from the pharmacy inventory; supervising the work results of support personnel; maintaining current registration; studying existing and new legislation; anticipating legislation; advising management on needed actions.
Protects patients and technicians by adhering to infection-control protocols and related procedures, rules, and regulations.
Maintains pharmacological knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
Performs all functions as a specialty staff pharmacist as required.
Acts as team lead to pharmacy technicians.
Develops and implements pharmaceutical care plans, which address high-risk side effects of medications.
Monitors drug interactions, therapeutic incompatibilities, and problems, ensuring safety of patients by documenting interventions.
Documents patient interactions in appropriate electronic medical record.
Protects patients and technicians by adhering to infection-control / hazardous medication protocols and related procedures, rules, and regulations.
Performs other related duties as required and directed.
Required:
PharmD degree
Completion of pharmacy internship and licensure in the State of Michigan.
One year of hospital/retail pharmacy experience preferred.
