As a Clinical Pharmacist you will conduct research and maintain or enhance clinical pharmacy programs such as drug therapy monitoring, health outcomes, disease state management, and care management. Employees in this position will work as an extension of the local Provider Care teams by aligning to geographical regions and clinics that care for a high volume of Medicare membership. The Clinical Pharmacist is responsible for supporting care management related pharmacy activities, drug therapy monitoring, and staff education, with a focus on improved health outcomes.

Collaborate with an integrated health care delivery team for pharmacy related practice quality improvement
Counsels patients and their care givers on safe and appropriate use of drugs
Conduct practice specific pharmacy data review to improve adherence to specified medications.
Available and acts as a clinical/pharmacy resource for the clinical support teams
Completing a comprehensive medication review of prescription and non-prescription medications with emphasis on patient assessment and counseling, safe and effective use of medications, identification of potential drug interactions, and other related activities
Assessing patients for medication adherence barriers and developing a plan to maximize patients' medication success through improving or maintaining adherence to medications
Developing a plan of care that details actions of the patient and/or prescriber based on the Pharmacotherapy assessment
Delivering high quality evidence-based therapeutic recommendations to providers
Documentation of clinical consultations including the plan of care into internal data base
Effectively participates in and contributes to staff meetings and other staff development opportunities and interdepartmental work groups
Identifies areas of improvement where systems modifications could result in operational advancements and efficiencies
Maintains a high level of clinical knowledge of chronic disease states and changes in pharmacy practice in adult and geriatric patient populations to serve as an authoritative information source on drugs and their utilization in therapy

Pharmacy degree, and active, unrestricted license to practice as a Pharmacist in the United States
Minimum of 3 years of experience as licensed Pharmacist
Experience providing clinical pharmacy services to Medicare patients (especially Parts C and D)
Highly motivated and works with minimal guidance
Excellent verbal and written communications skills in English
Strong computer skills: MS Office
Must have a reliable vehicle, current and unrestricted driver's license, and proof of auto insurance
Demonstrated ability to identify with a consumer in order to understand and align with their needs and realities.
Demonstrated ability to perform effective active listening skills to empathize with the customer in order to develop a trust and respect.

Ability/Willingness to travel up to 75% of the time

Health Plan (Managed Care) experience
MTM (Medication Therapy Management) Certification
Current Geriatric Licensure or in process of obtaining: CGP (Certified Geriatric Pharmacist)
BCPS or BCACP: Board Certified Pharmacotherapy Specialist or Board Certified Ambulatory Care Pharmacist
HEDIS and CMS star rating experience
