Summary:

Reports to Manager Ambulatory Clinical Pharmacy Services. Responsible for demonstrating clinical and operational knowledge and expertise as it relates to assessment treatment and care of ambulatory patients at Lifespan ambulatory sites.

Collaborates with interdisciplinary program leaders to deliver medication therapy management to ambulatory patients. Serves as a direct patient care clinician and ambulatory pharmacy practice program leader. Coordinates ambulatory pharmacy practice programs and medication use systems to ensure medication use is aligned with evidence-based best practices and regulatory standards.

Develops and executes quality initiatives to detect mitigate and prevent medication adverse events. Provides clinical pharmacy services in a variety of practice settings in collaboration with other pharmacists practitioners nurses and clinic personnel. Ensures ambulatory pharmacy practice programs are appropriately integrated throughout Lifespan ambulatory sites.

Serves as departmental clinical resource and liaison to practitioners pharmacists nurses and other clinic personnel. Works closely with peers and key stakeholders to establish infrastructure within the department and throughout the Lifespan system.

Responsibilities:

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Is responsible for knowing and acting in accordance with the principles of the Lifespan Corporate Compliance Program and Code of Conduct.

In addition to position specific responsibilities of a Clinical Pharmacist Specialist:

Coordinates development implementation and maintenance of ambulatory clinical pharmacy services to advance patient outcomes through the delivery of patient-centered care and establishment of strong collaborative pharmacist-patient relationships.

Facilitates the development and implementation of evidence-based treatment guidelines protocols and critical pathways as approved by appropriate committees.

Creates sustainable clinical pharmacist schedules in order to provide consistent pharmaceutical care to ambulatory clinic patients. Organizes and prioritizes work assignments.

Ensures ambulatory pharmacy services are provided in a timely manner are consistent with established team responsibilities reflect peer benchmarks and are provided with consideration of the aging process and human development stages (neonate child adolescent adult geriatric) and cultural patterns in each step of the care process.

Provides medication review reconciliation and patient counseling as appropriate.

Documents provided services in patient�s electronic medical record (EMR).

Supports continuum of patient care through prescription referrals to integrated outpatient pharmacies.

Develops and implements quality improvement measures to improve patient care and staff efficiency. Monitors outcomes. Develops working relationships with pharmacists attending physicians residents physician assistants nurse practitioners nurses and other health care team members.

Acts as a unifying voice and strong representative for clinical pharmacist team peers.

Participates in creation of a professional development plan for each clinical pharmacist team member in collaboration with Manager Ambulatory Clinical Services and employee.

Contributes to annual performance appraisal process for clinical pharmacist team members.

Interviews potential new clinical pharmacist team members. Coordinates training and on-boarding of new team members.

Precepts evaluates and serves as a mentor to pharmacy learners including PGY1 and PGY2 residents and PharmD students. Maintains professional development requirements set by American Society of Health-Systems Pharmacy (ASHP) for a qualified preceptor.

Participates in the development and presentation of education and training programs provided to medical nursing pharmacy and other staff.

Sustains professional competence and promotes development through participation in in-services workshops seminars and professional organizations.

Performs other duties as assigned.

Other information:

BASIC KNOWLEDGE:

Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy.

Successful completion of Post Graduate Year 2 (PGY-2) Residency from an accredited program in area of specialty if applicable and national board certification in area of specialty if available; otherwise completion of Post Graduate Year 1 (PGY-1) Residency from an accredited program and/or national Board Certification in Pharmacotherapy preferred. Clinical experience knowledge and development may be considered in lieu of formal training.

Maintains current unrestricted licensure as a licensed pharmacist in the State of Rhode Island.

Departmental certification in Medication Management (MTM) as applicable.

EXPERIENCE:

Effective interpersonal and communication skills both written and verbal to interact appropriately with patients families colleagues and others.

Effective organizational and presentation skills.

Knowledge of area-specific technology including but not limited to pharmacy information systems and electronic medical record.

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:

Extended periods of time spent standing and walking. Requires the visual and manual dexterity to operate a computer.

Occasional travel between facilities or sites is required.

INDEPENDENT ACTION:

Functions independently within a broad scope of departmental policies and practices.

Refers specific complex problems to Manager when clarification of Lifespan policies and procedures may be required.

None.
