Our caregivers# commitment is to make a positive difference in the lives of one another, our patients and our community by instilling our vision of Loving Kindness for all.# Newly developed Clinical Pharmacist position with focus on oncology, clinical program development and IV room oversite.# This is a full time, 40hrs/wk position.#Primary responsibilities involve processing/filling orders for drugs and prescriptions, monitoring patient drug therapies, providing drug information and maintaining appropriate inventory.# Pharmacists supervise and direct supportive personnel (technicians and clerks).# In the absence of the Pharmacy Director, you may be required to assume the essential responsibilities and perform duties of the Pharmacy Director.# Duties are performed in accordance with established policies, procedures, protocols, laws and regulations. Prepares and dispenses prescriptions and physician drug orders in the hospital (inpatient # outpatient) and retail (Apothecary) settings. Interprets drug orders (verbal and written) and transcribes them to computer patient profiles. Maintains accurate and complete pharmacy patient medication profiles. Dispense and compound pharmaceuticals. To include sterile products. Issues controlled substances maintaining all records as required by law and/or policy. Ensures safe, appropriate cost-effective drug therapies for patients. Monitors drug therapy regimens/profiles for contraindications, interactions (drug-drug and drug-food),#allergies and appropriateness of drug and dose. Assists with pharmacokinetics and drug dosing per protocols. Review/interpret antibiotic appropriateness and recommend changes, as necessary. Support patient care through monitoring and recommending appropriate pain management therapies. Reads and interprets information in patient charts/records appropriately. Detects and reports suspected Adverse Drug Reactions and errors accurately and in a timely manner. Supports the hospital drug formulary, minimizing procurement of non-formulary items, utilizing therapeutic substitution protocols and promoting rational drug therapy selection. Provide clinical consultation and clarification to practitioners.# Suggest appropriate, effective, safe, cost-effective, therapeutic alternatives to medical staff as needed. Provide accurate, timely drug information to the hospital professional staff. Provides drug education to patients and their families. Documents all clinical activities and interventions accurately and completely. Participates in the development and presentation of orientation, education and training programs for the medical staff, nursing service, other staff and the community. Contributes to the quality and effective operation of the Pharmacy service. Participate in the quality improvement and medication use review activities of the hospital and Pharmacy service.#Collects data, maintains logs, records and other documentation as assigned. Supervises and directs Pharmacy support personnel.# Oversees operation of the Pharmacy in the absence of the Director of Pharmacy. Helps keep the pharmacy areas clean, neat and well organized. Answers the telephone in an appropriate and timely manner.

Two years of Hospital and 1 year of retail Pharmacy experience preferred, but not required. Hospital residency preferred but not mandatory.
